The use of THA in patients ≥80 years of age with low CFS scores (<5) described here yielded favorable results and a relatively low rate of complications, but a well-controlled comparative study or randomized trial is required to further refine selection criteria for THA.

A biomechanical test on the fractured Femoral head showed that directly measured BMD has a significant correlation with the HB migration on the osteoporotic femoral head and thereby can be used to predict migration of lag screws.

Although the direct anterior approach in geriatric trauma patients is associated with a rather long learning curve, it is found to preserve the soft-tissues with no injury to abductors and shows an early advantage in elderly patients in terms of early recovery and therefore early possible full weight-bearing.

Medicine
Primary total hip arthroplasty compared with internal fixation appears to be a reasonably safe method of treating displaced fracture of femoral neck in elderly patients and an increased risk for intracapsular hip fracture developing nonunion with older age is shown.

Because of a higher intra-operative blood loss, an increased duration of the operation and a higher number of early and late dislocations, THR is not recommended as the treatment of choice in patients aged ≥ 70 years with a fracture of the femoral neck in the absence of advanced radiological osteoarthritis or rheumatoid arthritis of the hip.
