Barrett's esophagus (BE) is a premalignant condition. The incidence of adenocarcinoma in BE has been reported to be between 0.1-3%. Dysplasia in BE is patchy and extensive biopsy sampling is labor intensive, low yield and does not eliminate the sampling error completely. Volumetric laser endomicroscopy (VLE) is expected to enable endoscopists to do targeted biopsy of dysplastic/cancerous lesions (not visible on white light endoscopy) among patients with BE. We reviewed 7 studies with a total of…

Volumetric laser endomicroscopy in Barrett's esophagus: ready for primetime.
TLDR
Multiple newer advanced modalities have been developed to improve diagnostic abilities, including volumetric laser endomicroscopy (VLE), which utilizes second generation optical coherence tomography (OCT) to provide an enhanced circumferential image with the potential for improved diagnostic yield for dysplasia.
Barrett’s esophagus: novel strategies for screening and surveillance
TLDR
Various novel techniques for screening of Barrett’s esophagus such as unsedated transnasal endoscopy, cytosponge with trefoil factor-3, balloon cytology, esophageal capsule endoscope, liquid biopsy, electronic nose, and oral microbiome are presented.
Post-ablation buried neoplasia in Barrett’s esophagus
TLDR
A patient with BE and high-grade dysplasia successfully treated by EET who developed buried dysplastic BE during surveillance is presented.
Barrett's esophagus: The pathomorphological and molecular genetic keystones of neoplastic progression
TLDR
The results suggest that molecular, genetic and epigenetic alterations in the esophagus arise earlier than cancerous transformation, meaning the discussed targets can help form preventative strategies in at‐risk patient groups.
Recent advances in understanding and preventing oesophageal cancer
TLDR
Progress in the surveillance of Barrett’s oesophagus is explored, a pre-malignant condition for the development of Oesophageal adenocarcinoma, which means that more patients are considered at an earlier stage for potentially curative treatment.

TLDR
The evaluation and treatment of patients with Barrett's esophagus and early-stage neoplasia should be centralized in tertiary referral centers, where procedures are performed under optimal conditions, by expert endoscopists.
Volumetric laser endomicroscopy can target neoplasia not detected by conventional endoscopic measures in long segment Barrett’s esophagus
TLDR
Endoscopy with VLE can target neoplasia that cannot be localized by high-definition endoscope with NBI and random biopsies, and resulted in upstaged disease/diagnosed dysplasia that qualified the patient for endoscopic ablation therapy.
