Background The Manchester Triage System (MTS) determines an inappropriately low level of urgency (undertriage) to a minority of children. The aim of the study was to assess the clinical severity of undertriaged patients in the MTS and to define the determinants of undertriage. Methods Patients who had attended the emergency department (ED) were triaged according to the MTS. Undertriage was defined as a ‘low urgent’ classification (levels 3, 4 and 5) under the MTS; as a ‘high urgent…
The Manchester triage system: improvements for paediatric emergency care
TLDR
Modifications of the MTS for paediatric emergency care resulted in an improved specificity while sensitivity remained unchanged, and further research should focus on the improvement of sensitivity.
Improving the Manchester Triage System for Pediatric Emergency Care: An International Multicenter Study
TLDR
The Manchester Triage System after changing discriminators, and with the addition use of abnormal vital sign in patients presenting to pediatric emergency departments (EDs), performed slightly better than the original MTS.
Improving the prioritization of children at the emergency department: Updating the Manchester Triage System using vital signs.
TLDR
Six new discriminators based on vital signs lead to a small but relevant increase in performance and should be implemented in the MTS, with optimal cut-off value for each vital sign.
Manchester Triage System: main flowcharts, discriminators and outcomes of a pediatric emergency care 1
TLDR
Worried parents was the main flowchart used and recent events the most prevalent discriminator, comprising the hospitalization outcomes and permanency in observation in the pediatric emergency before discharge from the hospital.
Triage of children with headache at the ED: a guideline implementation study.
Initial assessment, level of care and outcome among children who were seen by emergency medical services: a prospective observational study
TLDR
Despite the incomplete use of all vital signs according to the RETTS-p, the EMS nurse assessment of children appears to be adapted to the clinical situation in most cases and the patients appear to be assessed to the appropriate level of care but indicating an over triage.
Accuracy of emergency department triage using the Emergency Severity Index and independent predictors of under-triage and over-triage in Brazil: a retrospective cohort analysis
TLDR
Despite rigorous and ongoing training of ESI users, a large number of patients in this cohort were under- or over-triaged, and advanced age, vital sign derangements, and specific chief complaints—all subject to limited guidance by the ESI algorithm—were particularly under-appreciated.

Validity of the Manchester Triage System in paediatric emergency care
TLDR
The MTS has moderate sensitivity and specificity in paediatric emergency care and specific modifications of the MTS should be considered in paediatrics emergency care to reduce overtriage, while maintaining sensitivity in the highest urgency categories.
Does the Manchester triage system detect the critically ill?
TLDR
The Manchester triage system (MTS) is a sensitive tool for detecting those who subsequently need critical care and are ill on arrival in the A&E department but did fail to detect some whom deteriorated after arrival in A &E.

TLDR
The most recent changes concerning paediatric triage assessment are highlighted and should be used in conjunction with the previous adult guidelines article.
Manchester triage system in paediatric emergency care: prospective observational study
TLDR
The Manchester triage system has moderate validity in paediatric emergency care, but errs on the safe side, with much more over-triage than under-Triage compared with an independent reference standard for urgency.

TLDR
The Manchester Triage System has a high sensitivity in assigning high priority (immediate/very urgent) to patients with acute coronary syndrome, and the combinations of flowchart and discriminators routinely used for triage have the potential to become more sensitive with no loss in specificity.

Triage of the pediatric patient in the emergency department: are we all in agreement?
TLDR
GEM participants were more likely to triage children with certain febrile illnesses at higher acuity levels as compared with their PEM counterparts, and the level of agreement of triage assignment within each group was only fair.
Detecting high-risk patients with chest pain.
TLDR
Nurses using the Manchester Triage System are a sensitive tool for identifying high risk cardiac chest pain but further work is required to assess whether additional training can improve sensitivity.
