In most cases monozygotic twins are concordant for genetic disease with rare exceptions. We report on a case of monozygotic twins discrepant for Beckwith–Wiedemann syndrome (BWS) phenotype with focus on molecular testing in the context of twin–twin transfusion. The propositus was one of male twins in a naturally conceived pregnancy to a 30-year-old gravida 2 para 2 mother. Delivery was at 28 weeks. There was no consanguinity or significant family history.
