Neuropathic pain is often resistant to opioids, so other medication classes, such as tricyclic antidepressants, anticonvulsants, and local anesthetics, are often used. Central sensitization, or pain 'wind-up', may perpetuate chronic neuropathic pain even when ongoing peripheral sensory input is absent. Wind-up is thought to cause allodynia, hyperalgesia, and hyperpathia. Receptors such as NMDA, AMPA, and M-glu have recently been identified for their role in central sensitization or pain 'wind…
Topical amitriptyline and ketamine for the treatment of neuropathic pain
TLDR
Topical amitriptyline-ketamine combination (AK) is a promising agent for peripheral neuropathic pain conditions and its role remains still controversial.
Ketamine Use for Cancer and Chronic Pain Management
TLDR
It is shown that ketamine is an effective analgesic in chronic pain conditions including cancer-related neuropathic pain and at the sub-anesthetic concentrations used for analgesia ketamine’s safety and adverse event profiles are much improved.
Compounded Ketamine A Useful Agent for Neuropathic Pain
TLDR
Topical analgesics differ from transdermal delivery methods in that prescribers use topical applications to deliver local, rather than systemic, effects.
Topical Treatments for Localized Neuropathic Pain
TLDR
This narrative review describes the available compounds for topical use in LNP syndromes and addresses their potential efficacy according to the literature, with relatively few systemic side effects or drug–drug interactions.
Topical and Peripheral Ketamine as an Analgesic

    Anesthesia and analgesia
  • 2014
TLDR
Topical analgesics are likely more effective in particular conditions (patient factors, disease factors), and future trials of topical ketamine should include a consideration of factors that could predispose to favorable outcomes.
Topical analgesics for neuropathic pain: Preclinical exploration, clinical validation, future development
TLDR
Topical analgesics have the potential to be a valuable additional approach for the management of neuropathic pain and there is interest in compounding topical analgesics for neuropathicPain, but several challenges remain for this approach.
Topic drug therapy for neuropathic pain
TLDR
Few studies in the literature validate the use of topic analgesics to treat neuropathic pain and, on the other hand, topic 5% lidocaine may be safely used with good analgesic effect and tolerability.
Management of Peripheral Neuropathic Pain

  • Medicine, Psychology
American journal of physical medicine & rehabilitation
  • 2005
TLDR
An overview of the cellular mechanisms associated with neuropathic pain is presented, the results of randomized, controlled trials with the major classes of available drugs are summarized, and treatment options that provide a rational basis for pharmacotherapy are discussed.

Longterm treatment of chronic neuropathic pain with the NMDA (N‐methyl‐D‐aspartate) receptor antagonist ketamine
TLDR
Two more case reports show promising results with longterm oral, subcutaneous or intramuscular treatment with ketamine or dextrometorphan, another clincically available NMDA-antagonist, and strengthens their conclusions that ketamine appears to have a specific pain-relieving effect in some types of chronic neuropathic pain.
Prolonged analgesic effect of ketamine, an N-methyl-D-aspartate receptor inhibitor, in patients with chronic pain.
TLDR
It is suggested that NMDA receptor-independent transmission in central pain pathways may contribute to the reduced efficiency of analgesic drugs often seen in chronic pain states.
Neuropathic pain in cancer patients: mechanisms, syndromes, and clinical controversies.
Four years' treatment with ketamine and a trial of dextromethorphan in a patient with severe post‐herpetic neuralgia
TLDR
Ketamine, a non‐competitive NMDA receptor antagonist, has in several case reports given pain relief but efficacy in dosages tolerated in long‐term ketamine treatment is unknown.

TLDR
It is apparent that nurses' concerns about life-prolonging treatment are persistent over time and across a variety of practice contexts, and these concerns are embedded in other persistent concerns about the inadequate involvement of patients and their family members in treatment decision-making processes and concernsAbout the structural and interpersonal characteristics of the work environment.
