Because cases of phenytoin toxicity during concomitant ticlopidine therapy have been reported, we investigated the effects of multiple doses of ticlopidine on phenytoin pharmacokinetics in six patients receiving phenytoin monotherapy. Two steady‐state dosing rate and serum phenytoin minimum concentration (Cmin) pairs were obtained for each patient administered oral phenytoin alone, then phenytoin plus 250 mg ticlopidine twice daily. All patients had serum Cmin ticlopidine values of 0.06 to 0.25… Expand
Concomitant use of mirtazapine and phenytoin: a drug–drug interaction study in healthy male subjects
TLDR
Co-administration of mirtazapine did not alter the steady-state pharmacokinetics of phenytoin and induction of CYP 3A3/4 was confirmed by the significantly increased 6β-hydroxycortisol/cortsisol ratio. Expand
Inhibition by ticlopidine and its derivatives of human liver cytochrome p450. Mechanism-based inactivation of CYP 2C19 by ticlopidine.
TLDR
It is shown that ticlopidine inhibits phenytoin clearance, which results in acute toxicity, and in vitro such inhibitory effects on bufuralol hydroxylation and on S-mephenytoin 4-hydroxylated have been shown on recombinant CYP 2C19. Expand
Inhibition of Cytochrome P450 (CYP450) Isoforms by Isoniazid: Potent Inhibition of CYP2C19 and CYP3A
TLDR
Inhibition of one or both CYP2C19 and CYP3A isoforms is the likely mechanism by which INH slows the elimination of coadministered drugs, including phenytoin, carbamazepine, diazepam, triazolam, and primidone. Expand
Clinical Significance of the Cytochrome P450 2C19 Genetic Polymorphism
TLDR
Genotyping for the common alleles of CYP2C19 before initiating PPIs for the treatment of reflux disease and H. pylori infection is a cost effective tool to determine appropriate duration of treatment and dosage regimens and, because of its narrow therapeutic range, genotyping of CYPs in addition to CYP1C9 may be needed to optimise the dosage of phenytoin. Expand
Pharmacogenetics, Drug-Metabolizing Enzymes, and Clinical Practice
TLDR
The current evidence base for pharmacogenetics in relation to drug-metabolizing enzymes is summarized and no other drugs have an evidence base that is sufficient to justify prospective testing at present, although some warrant further evaluation. Expand
Chloramphenicol Is a Potent Inhibitor of Cytochrome P450 Isoforms CYP2C19 and CYP3A4 in Human Liver Microsomes
TLDR
In conclusion, inhibition of CYP2C19 and CYP3A4 is the probable mechanism by which chloramphenicol decreases the clearance of coadministered drugs, which manifests as a drug interaction with chlorampshenicol. Expand
Risk and predictability of drug interactions in the elderly.
TLDR
There is a significant potential for drug interactions between AEDs and drugs commonly prescribed in geriatric patients with epilepsy. Expand
Pharmacological and clinical aspects of antiepileptic drug use in the elderly
TLDR
Evaluated AEDs are among the drug classes most commonly implicated as causing adverse drug reactions in an aged population; age alone is one of several contributors to alterations in AED response in the elderly; other factors include physical frailty, co-morbidities, dietary influences, and drug interactions. Expand
Inhibition of Cytochrome P450 Enzymes
Cytochrome P450s (P450s) are subject to inhibition/inactivation by various chemically diverse agents, which may interact directly or indirectly with either the P450 prosthetic heme or the proteinExpand

Phenytoin toxicity associated with ticlopidine administration.
A case of phenytoin sodium toxicity that appeared to be related to the administration of ticlopidine hydrochloride was recently observed at the Prescott Veterans Affairs Medical Center in Arizona.Expand
Individualization of phenytoin dosage regimens
TLDR
It was concluded that the use of individual pharmacokinetic parameters is practical and is also superior to the nomogram. Expand
Ticlopidine inhibition of phenytoin metabolism mediated by potent inhibition of CYP2C19
TLDR
Using human liver microsomes, it is shown that ticlopidine is a potent inhibitor of cytochrome P450 2C19, and the data emphasize the importance of CYP2C19 in the metabolism of phenytoin. Expand
Acute phenytoin toxicity followed by seizure breakthrough from a ticlopidine-phenytoin interaction.
TLDR
Because of the potential for acute and permanent adverse effects from a drug-drug interaction, phenytoin concentrations should be carefully monitored when beginning or ending ticlopidine therapy. Expand
Pharmacokinetic Drug Interactions with Phenytoin (Part I)
TLDR
Phenytoin has been reported to alter the pharmacokinetics of a large number of drugs, and current evidence suggests that the various cytochrome P450 isozymes may be affected differentially and, therefore, that the metabolic clearance of some drugs may be more sensitive to the enzyme-inducing effects of phenytoin. Expand
[Acute phenytoin poisoning secondary to an interaction with ticlopidine].
TLDR
The mechanism of action of ticlopidine at cytochrome P450 level, inhibiting the metabolic clearance of phenytoin, was responsible for this interaction between these drugs, and this case is the first to be published in Spain. Expand
Ticlopidine-Induced Phenytoin Toxicity
TLDR
Clinicians should be aware of this potentially serious drug interaction and either avoid the phenytoin–ticlopidine combination, or monitor closely for pheny toin toxicity. Expand
Clinical Pharmacokinetics of Ticlopidine
TLDR
It has been demonstrated that absorption of ticlopidine after oral administration is rapid, is improved when the drug is administered with food, but reduced by the coadministration of antacid, while bleeding time is determined by determination of the effect of the drug on bleeding time. Expand
Ticlopidine‐theophylline interaction
TLDR
The results seem to exclude direct liver toxicity and may suggest a reversible inhibition of the liver metabolic capacity of theophylline, a widely used bronchodilator drug. Expand
Pharmacokinetics of ticlopidine during chronic oral administration to healthy volunteers and its effects on antipyrine pharmacokinetics.
TLDR
Treatment with ticlopidine decreased antipyrine clearance, demonstrating that it inhibited drug-metabolizing enzymes, indicating that the interindividual variation in the pharmacokinetics of tic lopidine are explained by differences in metabolic clearance. Expand
