The varicocele remains the leading correctable cause of male infertility. This paper reviews the various pathophysiologic mechanisms which have been investigated in an attempt to explain the detrimental effect of the varicocele on spermatogenesis. While endocrinologic and toxic metabolic theories have not been well substantiated, the association of abnormal scrotal temperatures and abnormal spermatogenesis in some men with varicoceles has been reported by various investigators.
