}A series of 40 patients with the syndrome of normal-pressure hydrocephalus is presented. Diagnosis was based on clinical and computerized tomography (CT) scan criteria and was followed by the insertion of a ventriculoperitoneal shunt in every instance. All patients responded favorably to treatment (four of them had a fair outcome), and this response was maintained. There was one postoperative death, but no other serious complications. This study demonstrates that it is possible to diagnose the…

The author reviews the evolving understanding of the pathophysiology, diagnosis, and treatment of normal-pressure hydrocephalus and offers a strategy to assist in deciding which patients might benefit from shunting.

A 70-year-old female presented with the clinical triad of normal pressure hydrocephalus (NPH) and senile tremor and a ventriculoperitoneal shunt did not modify the clinical course except for a mild and transient improvement, and shunt malfunction occurred later on.

The patients with normal-pressure hydrocephalus had a later age and more frequent gait disturbance at the onset, shorter duration of the illness, rare signs of vascular disturbances, and more frequently severe mental deterioration.

The study illustrates the need to assess the pre-test probability of NPH based on combined clinical and CT data, before establishing the clinical usefulness of an ancillary test, with patients shunted for presumed normal pressure hydrocephalus.

Subdivision of NPH into an early stage and a late stage allows one to conclude prognostic evaluations about the course of the disease and the complete Adams triad should not be overestimated in differential diagnostic considerations.
Cognitive Recovery in Idiopathic Normal Pressure Hydrocephalus
- Medicine
- 1994
A clear postshunting improvement was seen in 96% of the patients at 1 year with a statistically significant correlation between high wave relative frequency and the grade of improvement (P < 0.05), and 66.6% of shunted patients showed a significant improvement in cognitive functions.
