ABSTRACT The General Health Questionnaire (GHQ-12) is a popular measure of psychological distress. Despite its widespread use, an ongoing controversy pertains to its internal structure. Although the GHQ-12 was originally constructed to capture a unitary construct, empirical studies identified different factor structures. Therefore, this study examined the dimensionality of the GHQ-12 in two independent meta-analyses. The first meta-analysis used summary data published in 38 primary studies…
Method effects associated with negatively and positively worded items on the 12-item General Health Questionnaire (GHQ-12): results from a cross-sectional survey with a representative sample of Catalonian workers
TLDR
Examining the presence of method effects, and their relationships with demographic covariates, associated with positively worded (PW) and/or NW items with GHQ-12 responses revealed that method effects were differentially related to both the sex and age of the respondents.
How different domains of quality of life are associated with latent dimensions of mental health measured by GHQ-12
TLDR
The findings confirm that the GHQ-12 as a multidimensional rather than unitary instrument measures distinct dimensions of mental health.
Understanding the population structure of the GHQ-12: evidence for multidimensionality using Bayesian and Exploratory Structural Equation Modelling from a large-scale UK population survey
TLDR
Novel Exploratory Structural Equation Modelling is used on a large, representative UK sample taken from the first wave of the Understanding Society Survey to exemplify the potential of traditional, restrictive assumptions to bias conclusions and policy recommendations and highlight the inferential limitations of using simple summed scores for mental health measurement.
Establishing norms for mental well‐being in young people (7–19 years) using the General Health Questionnaire‐12
TLDR
Overall, psychometric properties including sensitivity, suggest that the GHQ‐12 provides a robust indicator of short‐term mental state in children and adolescents.
Anxiety and depression in athletes assessed using the 12-item General Health Questionnaire (GHQ-12) - a systematic scoping review
TLDR
This review synthesised and compared studies using the GHQ-12 in athletes to inform future research by identifying trends and gaps in the literature and found the traditional scoring of 0-0-1-1 should be used with the cut-off set at ≥3.
Psychometric Properties of the Persian Version of the Personal Meaning Profile in Iran
TLDR
This study indicated that the Persian version of the PMP is an acceptable, reliable, and valid measurement to evaluate the meaning in life in Iranian medical sciences students.
Common mental disorders prevalence in adolescents: A systematic review and meta-analyses
TLDR
Global prevalence of CMD in adolescents was 25.0% and 31.0%, using the GHQ cut-off point of 4 and 3, respectively, pointing to the need to include mental health as an important component of health in adolescence and to theneed to include CMD screening as a first step in the prevention and control of mental disorders.
Health locus of control and mental health in patients with multiple sclerosis: Mediating effect of coping strategies.
TLDR
Patients with internal HLC were found to have positive mental health due to their problem-focused coping strategies, while patients with external HLC had worse mental health as they used more emotion-focused strategies.

A confirmatory factor analysis of the General Health Questionnaire‐12
The General Health Questionnaire-12 is a widely used screening instrument for detecting psychological strain in the general population. A literature search of the PsycInfo database extracted 167
Confirmatory factor analysis of the General Health Questionnaire (GHQ-12) in Spanish adolescents
TLDR
In an adolescent population from Southern Europe, the GHQ-12 showed high internal consistency and the factor structure that best fitted the data was the Graetz three-dimensional model, suggesting that the GHZ-12 should be used as a unidimensional scale, as currently done.

TLDR
The GHQ-12, when applied to a literate population in Africa, showed similar factor structure as in other regions of the world; hence, it can be recommended for use as a screening tool for general symptoms of psychological distress.
Factorial Validity of the General Health Questionnaire 12 in an Angolan Sample
Most evidence available has found multidimensional solutions for the General Health Questionnaire’s (GHQ12) structure, contrary to the authors’ original one-factor solution. Recently, the study of

TLDR
Analysis of the frequency of responses suggests that the response bias derives from the ambiguity of the response options for the absence of negative mood states, which is consistent with the GHQ-12 being a unidimensional scale with a substantial degree of response bias for the negatively phrased items.
Wording effects and the factor structure of the 12-item General Health Questionnaire (GHQ-12).
TLDR
The results showed additional evidence that the GHQ-12 has a unidimensional structure after controlling for method effects associated with negatively worded items, and found evidence for the spurious nature of the 3-factor solution in Graetz's (1991) model.
A comparative assessment of the factor structures and psychometric properties of the GHQ-12 and the GHQ-20 based on data from a Finnish population-based sample.
TLDR
Compared to the 12-item version, GHQ-20 provides additional fourth factor of anhedonia, suggesting some discriminative power, and analyses of the GHZ-20 suggested that the four-factor structure was superior to the three-factor model.
The factor structure of the GHQ-12: the interaction between item phrasing, variance and levels of distress
TLDR
Item phrasing, item variance and levels of respondents’ distress affect the factor structure observed for the GHQ-12 and may perhaps explain why different factor structures of the instrument have been found in different populations.
