A case is reported of a patient, age 40 yr, with progressive hypothalamic and visual failure. Autopsy revealed widespread multisystemic sarcoidosis with involvement of the central nervous system. The intracranial portion of the optic pathway was diffusely infiltrated by sarcoid granulomas.
