Recent studies have demonstrated that alterations in cardiac metabolism can be present in ischemic heart disease and heart failure, suggesting an increased utilization of non carbohydrate substrates for energy production, with a reduction in the efficiency of myocardial oxygen consumption. A direct approach to the manipulation of cardiac energy metabolism consists in modifying substrate utilization.
