One of the most important questions concerning diabetic nephropathy is the identification of its onset. By the time the clinical diagnosis is madewith the appearance of proteinuria exceeding 0.5 g/24 h, the renal lesions are advanced and irreversible by present therapeutic means. The onset of clinical nephropathy which progresses to end-stage renal failure cannot be identified by the appearance of the glomeruli.
