STUDY OBJECTIVES The objective of this study was to determine the impact of the timing of chest tube insertion on outcome for the treatment of empyema, using a new animal model of empyema. DESIGN A prospective, controlled randomized, blinded design was used. SETTING The study was conducted in an animal research laboratory. PATIENTS OR PARTICIPANTS Sixty-six 2- to 3-kg rabbits were used in this study. INTERVENTIONS After induction of empyema, the rabbits were divided into four groups…

Figures and Tables from this paper

The utility of daily therapeutic thoracentesis for the treatment of early empyema.
TLDR
It is concluded that therapeutic thoracentesis is at least as effective as early chest tube placement for the treatment of early empyema using the rabbit model of empyma.
Experimental pleural empyema model in rabbits: Why, how and what are the next steps.
TLDR
The creation of the experimental empyema model in rabbits was standardized to obtain a more empyemic pleural reaction and a model with two different human pathogen bacteria was created, which generated satisfactory results.
[Experimental empyema in rats through intrapleural injection of bacteria]
TLDR
It is possible to induce empyema in rats through Staphylococcus aureus pleural injection by a simple surgical technique, and differently from other experiments, the pleural injections of Pasteurella multocida did not provoke empyEMA in rats.
Video-assisted thoracic surgery for fibropurulent thoracic empyema: a bridge to open thoracic surgery.
TLDR
VATS for fibropurulentThoracic empyema is effective and less invasive, and it may be important as a bridge between minimally invasive and conventional open thoracic surgical management.
Experimental pleural empyema model in rabbits: Why, how and what are the next steps Eksperimentalni model empijema pleure kod kunia: zašto, kako i šta dalje
TLDR
With the intention to obtain a more empyemic pleural reaction, a model with two different human pathogen bacteria was created, which generated satisfactory results, but not as good as those contained in some of the reference literature data.
Thoracic empyema, application of video-assisted thoracic surgery and its current management
TLDR
A clinical algorithm supporting the early use of video-assisted thoracic debridement in the management of empyema in children is proposed and is associated with less pain and shorter recovery period than open thoracotomy.
Penetration of Clarithromycin in Experimental Pleural Empyema Model Fluid
TLDR
The levels of clarithromycin in the pleural fluid after intravenous administration are inhibitory for most of the usual pathogens causing empyema, which should be considered when macrolides are selected for the treatment of patients with em pyema.
Surgical versus non-surgical management of pleural empyema.

The Cochrane database of systematic reviews
  • 2002
TLDR
It would appear that for large, loculated pleural empyemas VATS is superior to chest tube drainage with streptokinase in terms of duration of chest tubes and hospital stay and there are questions about validity and the study is also too small to draw conclusions.
Penetration of newer quinolones in the empyema fluid
TLDR
Differences were found in the degree of penetration of the two quinolones into infected pleural fluid in rabbits and the clinical significance of these differences is unknown.
Research of Linezolid and Ertapenem in experimental parapneumonic pleural effusion
TLDR
Pleural fluid levels of both antibiotics are inhibitory for common specified pathogens causing empyema.

Serial pleural fluid analysis in a new experimental model of empyema.
TLDR
This model more closely mimics the empyema that occurs in humans, relative to previous animal models, and appears appropriate for additional randomized studies in which different methods for the treatment ofEmpyema can be evaluated.
Immediate drainage is not required for all patients with complicated parapneumonic effusions.
TLDR
Antibiotics alone were not sufficient in two cases which eventually required chest tube drainage, therefore, not all complicated parapneumonic effusions require drainage.
Empyema thoracis. Factors influencing morbidity and mortality.
TLDR
DC is more effective than OD in reducing morbidity and mortality when surgical intervention is necessary and DC was superior to OD in patients requiring major operation in total illness days.

Management of parapneumonic effusions. An analysis of physician practice patterns.
TLDR
Evaluation of physician practice patterns in managing complicated parapneumonic effusion and the impact of delaying thoracentesis or pleural drainage on duration of hospitalization, cost ofospitalization, and need for thoracotomy finds this practice pattern is associated with longer and more costly hospitalizations.
Management of parapneumonic effusions.
When a patient with a parapneumonic pleural effusion is first evaluated, a therapeutic thoracentesis should be performed if more than a minimal amount of pleural fluid is present. Fluid obtained at

Turpentine pleurisy in rabbits: a model of pleural fluid acidosis and low pleural fluid glucose.
TLDR
In the present model, after a single intrapleural injection of turpentine, pleural fluid acid generation made a contribution to pH in vivo, however, after reinjection it appeared that utiliza tion of glucose by metabolically active pleural surface cells was responsible for both the low glucose concentration and the increased lactate production and pleural fluids acidosis.
Diagnostic significance of pleural fluid pH and PCO2.
TLDR
The measurement of Pleural fluid pH is useful in the diagnosis and management of pleural effusions and has predictive value for the course of parapneumonic effusions.
