OBJECTIVES The primary aim of these guidelines was to evaluate the role of pharmacological agents in the treatment and management of paraphilia, with a focus on the treatment of adults males. [] Key Method To achieve this aim, an extensive literature search was conducted using the English language literature indexed on MEDLINE/PubMed (1990-2009 for SSRIs) (1969-2009 for antiandrogen treatments), supplemented by other sources, including published reviews.

The World Federation of Societies of Biological Psychiatry (WFSBP) 2020 guidelines for the pharmacological treatment of paraphilic disorders

The aim of these guidelines is to bring together different views on the appropriate treatment of paraphilic disorders from experts representing different countries in order to aid physicians in clinical decisions and to improve the quality of care.

Pharmacological treatment of patients with paraphilic disorders and risk of sexual offending: An international perspective

Although pharmacological agents can be seen as an intrusion into a patients’ sexual self determination, results indicate that ethical and clinical standards are being met in the majority of cases and further promotion of current WFSBP treatment guidelines would help to standardise practice across North American and European countries.

Antiandrogens, and mostly GnRH analogues, significantly reduce the intensity and frequency of deviant sexual arousal and behavior, although informed consent is necessary in all cases.

Current Concepts in the Pharmacotherapy of Paraphilias

This review of the literature provides suggestive evidence that paraphilias are well characterized disorders marked by pathological dimensions and available data on the use of selective serotonin reuptake inhibitors, steroidal antiandrogens and GnRH analogues strongly suggest the efficacy of these treatments for paraphilic disorders.

Paraphilias: Diagnostics, Comorbidities, and Treatment

Diagnostic criteria and guidelines for paraphilic disorders proposed by standardized diagnostic schemes (i.e., DSM-5, ICD-11) and current findings on the effectiveness of the diverse psychotherapeutic and pharmacological treatment strategies are discussed.

[Failure of a study in forensic psychiatric hospitals : Clinical trial to investigate the additive effect of triptorelin on the efficacy of psychotherapy].

The described facts illustrate how legal regulations that should protect vulnerable groups in medical research, in a specific case can lead to the fact that a therapy form relevant to the corresponding patient group cannot be scientifically investigated.

The Biological Treatment of Paraphilic Disorders: an Updated Review

These guidelines offer clinicians potentially useful, rational approaches to assessing treatment need in individuals with paraphilic disorders and suggest neuroimaging research suggests that functional magnetic resonance imaging may offer further promise in effectively assessing paraphilic Disorders to help direct treatment options.

The role of testosterone in sexuality and paraphilia--a neurobiological approach. Part II: testosterone and paraphilia.

From a neurobiological perspective an extended scientific proposal for design features to investigate the effects of antiandrogen treatment in large clinical trials is presented.

Validation of the Change or Stop Testosterone-Lowering Medication (COSTLow) Scale Using the Delphi Method Among Clinical Experts

The developed COSTLow-R Scale provides an instrument that can be used to structure the process of changing or discontinuing TLM in patients with severe paraphilic disorders.

Ethics and the Therapeutic Relationship in the Care of People Living with Paraphilic Disorders

The therapeutic relationship is the foundation of mental health practice and becomes the primary tool through which practitioners help elicit and sustain change in the lives of their patients. The

These practice guidelines for the biological treatment of substance use disorders were developed by an international Task Force of the World Federation of Societies of Biological Psychiatry to reach a consensus on a series of practice recommendations that are clinically and scientifically meaningful based on the available evidence.

Hormonal and psychopharmacological interventions in the treatment of paraphilias: An update

It is concluded that pharmacological strategies as part of a “complete” psychotherapeutic programme can make a limited, yet relevant contribution to the treatment of paraphilias.

Treatment of the paraphilic disorders: A review of the role of the selective serotonin reuptake inhibitors

The results seem to hold new promise for these alternative medications in the treatment of the paraphilic disorders and, possibly, in individuals with an inappropriately high sexual drive.

A comparison of treatment of paraphilias with three serotonin reuptake inhibitors: a retrospective study.

A retrospective study evaluates and compares the effectiveness of three SSRIs in a group of paraphilics and found that the severity of fantasies decreased and that there were no significant differences in the reported efficacy between fluvoxamine, fluoxetine, and sertraline.

The majority of patients described by the GP's were dependent drinkers, indicating that many at risk drinkers might perhaps not be correctly identified and should be carefully addressed by future GP-education programs.

Treatment of paraphilia in young adults with leuprolide acetate: a preliminary case report series.

This preliminary case series supports the conclusion that leuprolide deserves further examination as a potentially safe and effective component in the treatment of young adult patients with paraphilia.

Differential Pharmacological Treatment of Paraphilias and Sex Offenders

An algorithm for differential pharmacotherapy of paraphilias is developed and a protocol on side effects, contraindications, and monitoring of CPA and LHRH agonists is presented.

Pharmacotherapy of paraphilias in the next millennium.

Long-acting GnRH analogues, together with psychotherapy, are highly effective in controlling selected paraphilias (pedophilia, exhibitionism, and voyeurism), and are the most promising mode of therapy in the next millennium.
