}Lumbar myelographic defects consistent with herniated disc were found in 108 asymptomatic patients undergoing myelography for other reasons. Within 3 years, 64% of these patients developed symptoms of lumbosacral radiculopathy. The clinical features of these patients comprise a syndrome significantly different from that typically associated with classical lumbar disc herniation: the syndrome described here carries a much higher incidence of silent root compression with minimal pain. Incidental…

The data clears that sciatica can be caused by contralateral lumbar disk herniation and intervention only from the herniated side is sufficient.

Although MR imaging is a superb study when used in the search for degenerative disk disease and disk protrusion, conventional myelography is a crucial supplemental study that is necessary to confirm degenerative root impingement in the lateral recess as the cause of radiculopathy.

This prospective study was undertaken to determine whether preliminary CT could offer any information about disc degeneration and so reduce the demand on a MRI scanner and a useful algorithm for the investigation and assessment of patients with chronic low back pain is discussed.

Spinal CT scans and radiculograms of 100 patients who had undergone both examinations were studied with the aim of identifying morphologic CT features associated with compression of the intrathecal segment of the nerve root as demonstrated by Radiculography, and it proved to be possible to distinguish locations with CT features likely to correlate well the radiculate picture from those less likely to show good correlation.

It is believed that myelography should routinely precede surgery for lumbar disc lesions unless surgical treatment is being considered, and in cases in which operation for a lumbr disc lesion is clearly indicated on the basis of the clinical picture and lack of response to conservative treatment, a negative myelogram should not prevent the surgeon from exploring the lumbAR spinal canal.

A large series of myelograms is essentially unobtainable in individuals whose spinal subarachnoid space is known to be clinically normal, so the opportunity does arise, however, to survey the area of minimal defect with a total failure to expose any discernible disease.
