Individuals who develop schizophrenia often suffer long standing deficits. All too often available treatments remain palliative and do not improve the long-term course of illness. The neurobiological deficits associated with the onset of schizophrenia may be most active and damaging in the early stages of this life long illness, a fact which has shifted the focus of research and clinical work toward the early or prodromal stages of this disorder. Results from limited studies suggest that early…

This article reviews and describes measures that are currently being used and further developed in order to determine both the diagnostic criteria for the prodromic state and the severity of these putatively prodromal symptoms.

Cognitive therapy alone or in combination with low- dose atypical antipsychotics has been shown to be efficacious in reducing or delaying the transition to psychosis, as well as in ameliorating the severity of non-psychotic symptoms and distress.

It is concluded that a neurodevelopmental model of schizophrenia is supported by the data and that a range of novel treatment strategies may be neuroprotective by directly affecting the disorder's vulnerability core.
PROSPECTIVE OBSERVATIONS OF EMERGING PSYCHOSIS

A new, prospective view of the development of schizophrenia starting from the late prodromal phase of illness is offered, which focuses on the course of illness progression in three of eight cases who converted to schizophreniform psychosis while randomized on either active medication or placebo.
Cognitive functioning in the schizophrenia prodrome.
- Psychology, MedicineSchizophrenia bulletin

The results suggest that cognitive impairments may still be modest in the early stages of the initial schizophrenia prodrome and thus support current efforts to intervene in theearly course of impending schizophrenia because early intervention may prevent or delay the onset of frank psychosis and thus prevent further cognitive damage.

It is revealed that the risk of progression to psychosis was not reliably correlated with fulfilled criteria, but abscence of criteria credibly predicted the unlikelihood of psychosis emergence.

The SOPS positive symptom "conceptual disorganization" was found to be significantly related to disorganized behavior but not to other positive symptoms or to psychotic outcome, suggesting the importance of examining dimensions of psychopathology.

A hierarchy of symptomatic/syndromal progression in early manifest schizophrenia that may reflect active deterioration processes at work is suggested and it is suggested that any theory of schizophrenic pathophysiology must account for these patterns of symptom course.

The hypothesis that patients differ in the course of cognitive decline is supported and deterioration of function may follow the onset of overt psychosis in some patients is suggested; Prospective longitudinal studies of first-episode schizophrenic patients could directly test this hypothesis.

A reanalysis of the data indicates that early intervention with neuroleptics in first-break schizophrenic patients increases the likelihood of an improved long-term course and there is evidence that stable schizophrenia patients whose neuroleptic treatment is discontinued and have relapses may have a difficult time returning to their previous level of function.
