Stepped wedge designs (SWDs) have received considerable attention recently, as they are potentially a useful way to assess new treatments in areas such as health services implementation. Because allocation is usually by cluster, SWDs are often viewed as a form of cluster-randomized trial. However, since the treatment within a cluster changes during the course of the study, they can also be viewed as a form of crossover design. This article explores SWDs from the perspective of crossover trials…

Mixed-effects models for the design and analysis of stepped wedge cluster randomized trials: An overview
TLDR
This article provides a general model representation and regard various model extensions as alternative ways to characterize the secular trend, intervention effect, as well as sources of heterogeneity in the stepped wedge cluster randomized design.
Improving the Design and Analysis of Stepped-Wedge Trials
TLDR
This thesis provides a new formulation of a design effect for an SWT and introduces a novel method of analysis that excludes the within-cluster comparisons, which will lead to trialists using more efficient trial designs and more robust analysis methods.
Highly efficient stepped wedge designs for clusters of unequal size.
TLDR
Cluster-balanced designs, which allocate equal numbers of clusters to each sequence, are shown to have excellent statistical and practical properties and how the methods can be extended to the closed-cohort design is indicated.
Admissible multiarm stepped‐wedge cluster randomized trial designs
TLDR
This paper explains how the required sample size in these multiarm trials can be ascertained when data are to be analyzed using a linear mixed model, and describes how the design of such Trials can be optimized to balance between minimizing the cost of the trial and minimizing some function of the covariance matrix of the treatment effect estimates.
Impact of unequal cluster sizes for GEE analyses of stepped wedge cluster randomized trials with binary outcomes.
TLDR
It is concluded that the working independence assumption can lead to substantial efficiency loss and a large sample size regardless of cluster-period size variability in SW-CRTs, and recommend accounting for correlations in the analysis.
Information content of stepped-wedge designs when treatment effect heterogeneity and/or implementation periods are present.
TLDR
Stepped-wedge cluster randomized trials, which randomize clusters of subjects to treatment sequences in which clusters switch from control to intervention conditions, are being conducted with increasing frequency and require more complex statistical models to plan studies and analyze data.
Heterogeneous treatment effects and bias in the analysis of the stepped wedge design
The effect of an intervention in a stepped wedge design can vary across clusters or with time since exposure to treatment, but consequences of such heterogeneous treatment effects for the analysis of

TLDR
The tools provided by the theory of general balance are used to obtain an expression for the standard error of the estimated treatment effect in a general case where the assumed covariance structure includes random-effects at the time and time:cluster levels.
Variance formulae for multiphase stepped wedge cluster randomized trial.
TLDR
It is observed that the statistical test that uses data only relevant to a specific intervention has inferior power compared to the test when using all the study data, and power is reduced when both the total number of periods and lag are decreased simultaneously.
Analysis of stepped wedge cluster randomized trials in the presence of a time-varying treatment effect
Stepped wedge cluster randomized controlled trials are typically analyzed using models that assume the full effect of the treatment is achieved instantaneously. We provide an analytical framework for

Stepped-wedge cluster randomised controlled trials: a generic framework including parallel and multiple-level designs
TLDR
It is demonstrated that the impact of the ICC is likely to be smaller in a SW-CRT compared with a parallel CRT, especially where there are multiple levels of clustering, and through this unified framework, the efficiency of the SW- CRT and the parallelCRT can be compared.

The popularity of stepped wedge trials has increased since 2010, predominantly in high-income countries, and there is a need for further guidance on their reporting and analysis.

In large studies it is shown that the best design is a hybrid mixture of parallel and stepped‐wedge components, with the proportion of stepped wedge clusters equal to the cluster‐mean correlation.
