Abstract Primary correction of tetralogy of Fallot should be confined in most instances to those patients older than five years of age and in whom outflow patches may be avoided. Although most authors have favored the systemic pulmonary artery shunts for palliation of tetralogy of Fallot [15], direct pulmonary valve and/or right ventricular outlet procedures offer the advantage of good palliation, low operative risk, and better final physiologic results.
