}A 54-year-old female presented with spontaneous thoracic spinal cord herniation manifesting as chronic progressive motor weakness in both legs. Spastic paraparesis (4/5) and pathological reflexes such as ankle clonus were noted. She also had mild bladder dysfunction but no bowel dysfunction. She had no sensory disturbance, including tactile and pinprick sense. Magnetic resonance (MR) imaging revealed that the atrophic spinal cord was displaced into the ventral extradural space at the T4-5…

This is the first reported case of a spontaneous ventral extradural spinal meningeal cyst causing cord compression and Cyst-peritoneal shunt should be considered in cases in which complete resection of the cyst is made more difficult or risky by the need of more aggressive surgical maneuvers.
Idiopathic Spinal Cord Herniation Associated With a Thoracic Disc Herniation
- MedicineClinical spine surgery
- 2020
Although ISCH is a rare clinical condition that causes myelopathy, patients managed with surgery generally, though not universally, have a favorable neurological outcome, and an operative technique used to treat this condition is demonstrated.
Spinal Cord Herniation: Why Anterior Thoracic?
- Medicine
- 2015
The role of an anterior pulling force by the Hofmann (meningo-vertebral) ligaments is surmised; acting when the physiologic thoracic kyphosis suddenly increases; it allows the nipping/ protrusion of the spinal cord.
Idiopathic spinal cord herniation: diagnostic, surgical, and follow-up data obtained in five cases.

Based on the results and complications in these five cases, the authors now believe that ISCH should be treated by using a dural patch to close the dural defect at the site of the herniation.
Idiopathic Thoracic Spinal Cord Herniation: Report of 10 Patients and Description of Surgical Approach

Iiopathic spinal cord herniation presents with symptoms and signs of spinal cord dysfunction and a very characteristic imaging appearance, and can be treated safely, with the expectation of neurological stabilization and some improvement.

A unilateral dorsolateral, transpedicular approach combined with laminectomy provides excellent exposure for ventral or ventrolateral dural defects associated with idiopathic spinal cord herniation and minimizes spinal cord manipulation.

Patients presenting with a diagnosis of posterior intradural arachnoid cyst should be evaluated carefully for the presence of an anterior spinal cord herniation, and offering surgery to patients even when neurological compromise is advanced is recommended.

- MedicineJournal of neurosurgery

The authors suggest modifying the classification by Nabors, et al., of spinal meningeal cysts to include this mechanism of spinal cord herniation based on abnormal positioning of the spinal cord in the dural sleeve and the known anteroposterior movements of the cord that occur with cardiac and respiratory pulsations.

The patients showed gradual improvement postsurgery, as did most of the patients in the previously reported cases, and the CSF flow and endoscopic studies were found to be particularly informative, and dural patching with surgical membrane is recommended.

The authors postulate that spinal cord herniation occurred for the following reason: the pressure of the intradural arachnoid cyst on the dorsal aspect of the spinal cord caused thinning of the dura, leading to a tear and, thus, the development of an extradural arACHnoid Cyst.
