Introduction: Gamma-hydroxybutyrate (GHB) is a short-chain fatty acid that is synthesized within the CNS, mostly from its parent compound gamma amino butyric acid (GABA). GHB acts as a neuromodulator/neurotransmitter to affect neuronal activity of other neurotransmitters and so, stimulate the release of growth hormone. Its sodium salt (sodium oxybate: SXB) was approved by the Food and Drug Administration (FDA) for the treatment of narcolepsy. SXB has shown to improve disrupted sleep and…
Gamma-Hydroxybutyrate (Sodium Oxybate): From the Initial Synthesis to the Treatment of Narcolepsy–Cataplexy and Beyond
GHB improved night sleep quality and reduced daytime sleepiness andcataplexy without development of tolerance or significant side effects in narcolepsy and later reports have shown positive clinical effects in Parkinson's disease, chronic fatigue syndrome and fibromyalgia.
The Impact of Growth Hormone and Gamma-Hydroxybutyrate (GHB) on Systems Related to Cognition
GHB treatment created an imbalance regarding the opioids MEAP/DYNB and increased the levels of MEAP significantly in regions of the brain that are of importance for the development of drug dependence.
Sodium Oxybate as Off-label Treatment for Anxiety Disorder: Successful Outcome in a Low-energy Anxious Resistant Patient
A Spanish patient affected by resistant anxiety disorder was successfully treated with SO after an unsuccessful treatment with benzodiazepines and antidepressants, and it is shown that SO can be a useful and promising tool in the treatment of anxiety disorder.
An update on pharmacotherapy for the treatment of fibromyalgia
None of the currently available drugs are fully effective against the whole spectrum of fibromyalgia symptoms, namely pain, fatigue, sleep disturbances and depression, among the most relevant symptoms.
Fibromyalgia Syndrome and Sleep
The high frequency of sleep alterations and their complex relationship with other manifestations in fibromyalgia mean that physicians should specifically address them, in order to have a positive effect on patients’ quality of life.
Fibromyalgia Pathogenesis and Treatment Options Update
The most recent diagnostic criteria for fibromyalgia, as put forth by the American College of Rheumatology, will be summarized, and the evidence behind non-pharmacologic treatments, including massage therapy, exercise, and acupuncture, are discussed.
Monotherapy or Combination Therapy for Fibromyalgia Treatment?
Fibromyalgia is a chronic pain disease whose clinical symptomatology also includes different symptom domains: fatigue, sleep disturbances, morning stiffness, dyscognition, and psychological distress, which requires well-designed clinical trials exploring specific drug combinations selected on the basis of potential additive or synergistic effects.
The mirogabalin ALDAY phase 3 program in pain associated with fibromyalgia: the lessons learned

The main lessons learned from ALDAY were the need for a comprehensive patient-focused strategy to preliminarily identify the challenges of fibromyalgia based on patient perspective and study complexity, and a harmonized, truly patient-centric, global regulatory guidance accepted by regulatory agencies.
The role of sleep and cytokines in the development of mechanical hypersensitivity in a mouse model of musculoskeletal sensitization
The consequences of musculoskeletal pain include sleep disruption, an observation that has been made in the clinical literature but has yet to be studied using a model in which mechanical hypersensitivity was induced by injection of acidified saline into muscle.
Association between alcohol consumption and symptom severity and quality of life in patients with fibromyalgia
It is demonstrated that low and moderate alcohol consumption was associated with lower fibromyalgia symptoms and better QOL compared to no alcohol consumption, and future studies should examine whether alcohol could have a salutary effect on pain and other symptoms in fibromy arthritis.

gamma-Hydroxybutyrate/sodium oxybate: neurobiology, and impact on sleep and wakefulness.
Evidence suggests a role for GHB as a neuromodulator/neurotransmitter, and sodium oxybate has been observed to modulate sleep in nonclinical study participants, and sleep and wakefulness in clinical populations, including groups with insomnia, fibromyalgia and narcolepsy.
The effects of sodium oxybate on clinical symptoms and sleep patterns in patients with fibromyalgia.
Sodium oxybate effectively reduced the symptoms of pain and fatigue in patients with FM, and dramatically reduced the sleep abnormalities associated with the nonrestorative sleep characteristic of this disorder.
γ-Hydroxybutyrate/Sodium Oxybate
Sodium oxybate has been observed to modulate sleep in nonclinical study participants, and sleep and wakefulness in clinical populations, including groups with insomnia, fibromyalgia and narcolepsy, and evidence suggests a role for GHB as a neuromodulator/neurotransmitter.

The subjective quality of night sleep improved in all patients and the number of irresistable daytime attacks of sleep and cataplexy substantially diminished, and improvement has been maintained for up to 20 months without the development of tolerance.
The clinical development of gamma-hydroxybutyrate (GHB).
GHB was being developed for the treatment of narcolepsy, leading to the approval of Xyrem (sodium oxybate) oral solution in 2002, and post-marketing surveillance indicates sodium oxybates has an acceptable safety profile and presents minimal risk for the development of physical dependence.
Flumazenil effects on growth hormone response to gamma‐hydroxybutyric acid
GHBA induced a significant increase in GH plasma levels; flumazenil pretreatment antagonized GHBA action on GH secretion, suggesting a subpopulation of GABA receptors or GHBA-specific receptors seems to be involved in GHBAaction.
γ-Hydroxybutyric Acid
Investigation of the inborn error of metabolism succinic semialdehyde deficiency and the murine model of this disorder (SSADH knockout mice), in which GHB plays a major role, may help dissect out GHB- and GABAB receptor-mediated mechanisms.
The Effects of Nabilone on Sleep in Fibromyalgia: Results of a Randomized Controlled Trial
Nabilone is effective in improving sleep in patients with FM and is well tolerated, and low-dose nabilone given once daily at bedtime may be considered as an alternative to amitriptyline.
