Intensive glycemic control results in long-term reduction of diabetic retinopathy, although initial short-term worsening of retinopathy is not uncommon. The authors report a patient with a history of stable diabetic retinopathy who, despite achieving abrupt glycemic control following laparoscopic Roux-en-Y gastric bypass surgery, suffered rapid worsening of her retinopathy within 1 month of surgery.
