The digitalics (digoxin and digitoxin) have an estrogenic action, confirmed by the presence of gynecomastia observed in men who are users of steroids, and it is also confirmed by the increase of vaginal trophism in postmenopausal female users, although little is known about the endometrium. These effects might result from the similarity of the chemical structures between the digitalics and estrogens. This study evaluated 27 female mice. Twenty-four mice were oophorectomized and in three of them…

This pathological case study is about a 54-years male patient who received digoxin & spironolactone as a treatment of choice in dilated cardiomyopathy and is suspecting gynecomastia due to spironlactone and digoxin as the pathological cause.

It is concluded that digitoxin probably acts directly as a weak estrogen and that this effect probably explains the estrogen-like side effects seen with digitoxin therapy in man.

The structural similarity of the digitalis aglycones to the cyclopentanoperhydrophenanthrene radical of the steroidal hormones raised the question whether digitalis was itself responsible for the breast changes or whether hepatic insufficiency incident to myocardial failure or nutritional factors might have been the underlying causes.
