Interpretation of renin-angiotensin blockade with angiotensin converting enzyme inhibitors is potentially confounded by their multiple effects. We used a selective renin inhibitor (enalkiren, A- 64662) to explore the renal and endocrine effects of angiotensin II in healthy men. Each received 90-minute enalkiren infusions at 2-day intervals, on a low (10 mmol, 16 subjects) and high (200 mmol, 12 subjects) salt diet Plasma renin activity, immunoreactive plasma angiotensin II and aldosterone…

Figures and Tables from this paper

Responses to converting enzyme and renin inhibition. Role of angiotensin II in humans.

The results indicate that the acute renal response to captopril largely reflects a reduction in Ang II formation, which is similar to that seen in healthy men on a 10-mmol sodium diet.

Renal hemodynamic response to an angiotensin II antagonist, eprosartan, in healthy men.

The renal vasodilator response to the angiotensin II antagonist eprosartan closely resembles responses to renin inhibition and exceeds previously reported responses to ACE inhibitors.

Intrarenal Angiotensin II Formation in Humans Evidence From Renin Inhibition

Abstract The intrarenal production of angiotensin II (Ang II) as a local hormone, suggested by multiple lines of investigation, has been difficult to buttress with evidence of functional significance

AT1-receptor blockade and the kidney: importance of non-ACE pathways in health and disease

Comparison of the renal vascular responses to captopril and candesartan shows a strong correlation between the effects of ACE inhibition and AT1-receptor blockade, indicating that the deleterious effects of renin-angiotensin system activation in diabetes are mediated largely through angiotens in II.

Disparity between blood pressure and PRA inhibition after administration of a renin inhibitor to anesthetized dogs: methodological considerations.

The data support the conclusion that BP relates better to PRA-AT than P RA-C, and the dissociation sometimes observed in studies with renin inhibitors between changes in BP and PRA may be attributed to the assay used to determine PRA.

Comparative Pharmacological Interruption of the Renin System Pathways for Angiotensin II Generation in Intact Human Tissue : Evidence From Print

Findings indicate that in the intact human kidney, virtually all Ang II generation is renin-dependent but at least 40% of Ang I is converted to Ang II by pathways other than ACE, presumably a chymase, although other enzyme pathways exist.

Pathways for angiotensin II generation in intact human tissue: evidence from comparative pharmacological interruption of the renin system.

Findings indicate that in the intact human kidney, virtually all Ang II generation is renin-dependent but at least 40% of Ang I is converted to Ang II by pathways other than ACE, presumably a chymase, although other enzyme pathways exist.

Renal circulation and blockade of the renin-angiotensin system. Is angiotensin-converting enzyme inhibition the last word?

The intriguing possibility that the undoubted therapeutic efficacy of ACE inhibition in renal injury, documented most rigorously for type I diabetes mellitus, might be exceeded with the newer classes of agent is raised.

Hemodynamic and Humoral Effects of the New Renin Inhibitor Enalkiren in Normal Humans

There was a close correlation between plasma renin activity and plasma angiotensin II levels and active and total renin measured directly by monoclonal antibodies rose in dose-related fashion in response to renin inhibition.

Blockade and stimulation of renal, adrenal, and vascular angiotensin II receptors with 1-Sar, 8-Ala angiotensin II in normal man.

P113 is a partial agonist in normal man, inducing an angiotensin-like response in settings in which endogenous A II is not playing a tonic role, and displaying dominant antagonist activity in settingsin which AII is active.

Defect in the sodium-modulated tissue responsiveness to angiotensin II in essential hypertension.

A substantial subgroup of essential hypertensives has an abnormality in responsiveness to AII in two systems central to volume homeostasis: the kidney and adrenal, which could contribute to the elevated blood pressure.

Intrarenal renin inhibition increases renal function by an angiotensin II-dependent mechanism.

The results indicate that intrarenal angiotensin II acts as a physiological regulator of renal sodium and fluid homeostasis.

Control of arterial pressure and renal function during chronic renin inhibition.

CP-71362 is a potent inhibitor of dog plasma renin, and there is no waning of this inhibitory effect of CP-713 62's hypotensive actions over 7 days.

Renal vasodilatation after inhibition of renin or converting enzyme in marmoset.

The results indicate that the renal vasodilatation induced after converting-enzyme or renin inhibition is mainly due to decreased formation of angiotensin II.

The systemic and renal haemodynamic profiles of U-71038 and captopril appear to be similar, suggesting that renin and ACE inhibition elicit fundamentally similar cardiovascular effects in conscious sodium-depleted cynomolgus monkeys via a decreased formation of angiotensin II (Ang II).

Hemodynamic and Biochemical Consequences of Renin Inhibition by Infusion of CGP 38560A in Normal Volunteers

The measurement of plasma renin activity cannot replace that of plasma angiotensin II to evaluate the efficacy of renin inhibitors and, in conclusion, CGP 38560A was well tolerated.

Correction of abnormal renal blood flow response to angiotensin II by converting enzyme inhibition in essential hypertensives.

Converting enzyme inhibitors may reduce blood pressure specifically in this subset of patients with essential hypertension, who are sodium sensitive by way of mechanisms more closely related to local than systemic activity of the renin-angiotensin system.

There is no correlation between inhibition of PRA and the reduction in blood pressure in chronically sodium-deficient dogs, and in other studies comparing renin inhibition with angiotensin converting enzyme (ACE) inhibition, there was evidence for greater efficacy of ACE inhibition in conscious sodiumdeficient Dogs, but no evidence of any difference in one-kidney, one clip hypertensive dogs.
