OBJECTIVE Surgical error is common and contributes to complications for patients, necessitating detailed prospective collection and analysis of error data that emphasizes prevention. METHODS One neurosurgeon prospectively recorded errors and complications for consecutive patients undergoing elective neurosurgical procedures.
