Introduction With the advancements in diagnostic and therapeutic upper gastrointestinal endoscopy (UGIE), clear mucosal visualization is essential to ensure optimal outcomes. Though routinely followed in Japan and Korea, pre-endoscopic preparation is seldom used in India. We evaluated the efficacy of a pre-endoscopic drink of N-acetylcysteine (NAC) and simethicone in improving mucosal visibility during UGIE. Patients and Methods This study was a retrospective, investigator blind study with a…

Minimizing the Risks of Simethicone in Endoscope Reprocessing

This paper was written as a companion to the new guidelines with a focus on minimizing the risks of simethicone in endoscope reprocessing.

Endoscopy Flashback 2021: Thanking the Authors and Reviewers of the Journal of Digestive Endoscopy

Journal of Digestive Endoscopy has completed one more successful year with its impressive endoscopy-based articles and congratulates the following authors for their best contribution in endoscopic research in 2021.

Premedication with N-acetylcysteine and simethicone improves mucosal visualization during gastroscopy: a randomized, controlled, endoscopist-blinded study

Whether the use of a premedication solution containing the mucolytic agent N-acetylcysteine and the surfactant simethicone improves mucosal visualization within a UK diagnostic gastroscopy service is determined.

Simethicone for the Preparation before Esophagogastroduodenoscopy

Administration of simethicone prior to EGD reduces the amount of gastric foam and bubbles and provides better visibility for evaluating the mucosa and decreases the duration of endoscopy.

Pre-medication to improve esophagogastroduodenoscopic visibility: a meta-analysis and systemic review.

There is improved visibility with pre-medication using Simethicone before esophagogastroduodenoscopy, and administration of Pronase or N-acetyl-Lcysteine may be of little use in improving visibility.

A prospective randomized controlled trial comparing simethicone, N-acetylcysteine, sodium bicarbonate and peppermint for visualization in upper gastrointestinal endoscopy

The application of simethicone plus NAC is safe, improves endoscopic visualization and requires a minimal amount of mucosal cleansing solution, and the addition of sodium bicarbonate and peppermint further improved visualization for the upper and lower gastric body.

Efficacy of premedication with activated Dimethicone or N-acetylcysteine in improving visibility during upper endoscopy.

Premedication with activated Dimethicone 20 min prior to the upper endoscopy leads to the best visibility, and N-acetylcysteine does not improve visualization by itself.

Improving quality of colonoscopy by adding simethicone to sodium phosphate bowel preparation.

The addition of simethicone is of benefit for colonoscopic bowel preparation by diminishing air bubbles, which results in enhanced visibility and endoscopist and patient satisfaction is also increased.

Endoscopic and enzymatic treatment of gastric bezoar with acetylcysteine.

A 40-year old woman presented with a 2-week history of nausea, early satiety and eructation and was fitted with a endoscopic retrograde cholangiopancreatography catheter for instillation of acetylcysteine, which softens the bezoars allowing an easier and faster fragmentation by mechanical means.
