Objective The perioperative management of patients undergoing otolaryngologic procedures is increasingly complicated by the use of newer antithrombotic agents. Furthermore, with advances in anesthesia and surgical technique, otolaryngologists are presented with the challenge of operating on patients with advanced comorbid diseases. The objective of this review is to provide evidence-based recommendations on perioperative antithrombotic management for common otolaryngologic procedures. Data…
AHNS Series: Do you know your guidelines? Perioperative antithrombotic management in head and neck surgery
TLDR
Although elective procedures may be delayed for optimization of antithrombotic medication, emergent procedures cannot and the decision is made more difficult when patients with moderate‐to‐high risk for thromboembolic events undergo procedures that are high risk for bleeding.
[Prevention of thromboembolic complications in otorhinolaryngological surgery].
TLDR
Current clinical guidelines and recent publications on the perioperative prophylaxis of venous thromboembolism in ENT-HNS patients and foreign sources and national guidelines on diagnosis, treatment, and prevention of VTE are reviewed.
Dental management of patients on anti-thrombotic agents

TLDR
The purpose of the paper is to optimize the management of the dental patients on anti-thrombotic medication via standardization of treatment protocol of such a patient.
The international normalised ratio predicts perioperative complications in revision total hip arthroplasty.
TLDR
A direct relationship between specific preoperative INR levels and poor outcomes following rTHA, including increased LOS, transfusion requirements and infection is illustrated, and current INR guideline targets may need to be re-examined when optimising patients for revision arthroplasty.

A systematic review and synthesis of the English-language literature examining the perioperative management and outcomes of patients receiving long-term OAC therapy found major bleeding while receiving therapeutic OAC was rare for dental procedures.
The perioperative management of antithrombotic therapy: American College of Chest Physicians Evidence-Based Clinical Practice Guidelines (8th Edition).
TLDR
The key recommendations in this article include bridging anticoagulation with therapeutic-dose subcutaneous (SC) LMWH or IV UFH over no bridging during temporary interruption of VKA therapy and continuing aspirin and clopidogrel in the perioperative period.
Tonsillectomy in the Anticoagulated Patient
TLDR
Tonsillectomy can be done relatively safely in an anticoagulated patient at high risk for thrombosis and the perioperative bridging strategy should account for its unique risk of primary and secondary postoperative hemorrhage.
How I treat anticoagulated patients undergoing an elective procedure or surgery.
TLDR
Periprocedural data with the novel oral anticoagulants, such as dabigatran, rivaroxaban, and apixaban, are emerging, but their relatively short half-life, rapid onset of action, and predictable pharmacokinetics should simplify peripROcedural use.
Perioperative management of antithrombotic therapy: Antithrombotic Therapy and Prevention of Thrombosis, 9th ed: American College of Chest Physicians Evidence-Based Clinical Practice Guidelines.
TLDR
Perioperative antithrombotic management is based on risk assessment for thromboembolism and bleeding, and recommended approaches aim to simplify patient management and minimize adverse clinical outcomes.
Oral anticoagulation in surgical procedures: risks and recommendations
TLDR
It is concluded that routine surgery in anticoagulated patients yields a high perioperative bleeding and thromboembolic risk, with the highest risk in thoracic surgery.

TLDR
It is suggested that preoperative treatment with fondaparinux for NSTE-ACS is as safe as enoxaparin in terms of postoperative bleeding and transfusion needs.
Airway Complications in Thyroid Surgery
TLDR
Perioperative and postoperative morbidity and mortality were studied in a series of 3,008 thyroidectomies and the recurrent laryngeal nerve, which may have been affected preoperatively, was found to be damaged postoperatively.
