}BACKGROUND
There is a strong inverse relationship between outcome in patients with acute ischemic stroke from emergent large vessel occlusion (ELVO), and time to reperfusion from intra-arterial therapy. [] Key MethodRESULTS
MSTU was dispatched for reported new onset of right hemiparesis in a patient. Teleneurological assessment detected findings consistent with a severe left middle cerebral artery (MCA) syndrome. Noncontrast CT head revealed left lenticulostriate hypoattenuation.

Abbreviated pre-hospital stroke scales can be used to guide EMS personnel in quickly deciding if a patient is undergoing a stroke, and the expanded window of endovascular treatment for acute ischemic stroke patients to within 24 h from symptom onset brings patient screening and triaging to greater importance.
