Objective: To management hypertension care in Non-communicable disease clinic for the quality of care. Design and Method: This study aimed to develop Hypertension care model in Non -communicable disease clinic for Khiri Mat hospital, Sukhothai Province of Thailand. The framework used to B.I.C3.S Model for management in hypertension care. B.I.C3.S Model includes over body mass index care (>30), impaired fasting glucose care, chronic kidney disease care, cardiovascular risk, chronic cardiovascular care, smoking and alcohol drinking care. The study was conducted during October 2015 to December 2015. Results: 1 Hypertension patients can control blood pressure (less than 140/80 mmHg) 93.3% 2 Hypertension patients are diagnosed with new Diabetes mellitus 7 persons. 3 Hypertension patients who had a body mass index greater than 30 had been monitoring in Non-communicable disease clinic 43 persons. 4 Hypertension patients can stop smoking 5.19%. 5 Hypertension patients can stop drinking 3.45%. 6 Hypertension patients with chronic cardiovascular disease had received continuing care in Non-communicable disease clinic 81.91%. 7 Hypertension patients with chronic kidney disease had received continuing care in Non-communicable disease clinic 23.83%. Conclusions: Before implementing this model at Khiri Mat hospital, involved persons had a meeting to ensure that the model was applied appropriately and effectively. Other hospitals may apply this model, but they need to consider the expectations of all users, service providers, and community services as well as the context of their hospital.
