We reported a term, 3.7 kg baby boy, who was admitted to the Neonatal ward at day 4 of life for fever and abdominal distension. He continued to have persistent high grade fever, progressive massive hepatosplenomegaly with severe neonatal hepatitis, pancytopenia and poor weight gain. Multiple blood and urine cultures were negative for bacterial and fungal but the C-reactive protein were elevated > 100mg/dL. There were no response to empirical antibiotics and antifungal therapy.
