Gall-stone intestinal obstruction (GSO) is an unusual form of mechanical obstruction and a rare complication of cholelithiasis. The treatment options are controversial, usually the management is surgical but associated with significant morbidity and mortality. A spontaneous evacuation of the gall-stone that had induced GSO is even more exceptional, only few reports being published up to date. We report the case of an 81-year-old female patient presenting GSO admitted to our department due to…
Gallstone ileus: a rare cause of intestinal obstruction -- case report and literature review.

Acute intestinal obstruction due to gallstone ileus.

Gallstone ileus should be suspected in the elderly with SBO symptoms and surgical treatment must be individualized for each case to reduce post-operative complications.

A 69-year-old gentleman presented with symptoms and signs of SBO, and a CT scan revealed that the stone had passed into the rectum and was spontaneously evacuated.
Comparison of Surgical Treatments of Gallstone Ileus: Preliminary Report
TLDR
The purpose of this study was to compare the two investigated surgical procedures: treatment of intestinal obstruction alone or combined with urgent cholecystectomy and fistula repair, and simple enterotomy should be the procedure of choice for patients with gallstone ileus.
Management of gallstone ileus.
TLDR
It seems that a one-stage procedure (enterolithotomy plus fistula repair and cholecystectomy), when feasible, should be the first choice in elderly patients with gallstone ileus.
Role of helical CT in diagnosis of gallstone ileus and related conditions.
TLDR
Helical single-detector and MDCT may improve the diagnosis of gallstone ileus, providing important information regarding the exact number, size, and location of ectopic stones and the site of intestinal obstruction or direct visualization of a biliary-enteric fistula, to help clinicians in the therapeutic management of patients.
Gallstone Ileus: Diagnosis and Management
TLDR
Management of gallstone ileus must be individualized and the one-stage procedure should be offered only to highly selected patients with good cardiorespiratory reserve and with absolute indications for biliary surgery at the time of presentation.

Advantages of one-stage repair in case of gallstone ileus.
TLDR
The results prove that the concomitant one-stage repair for gallstone ileus significantly reduces the overall mortality rate compared to the single enterolithotomy alone and later cholecystectomy based on patient symptoms.
