Inaccurate, missing, incomplete, or out of data documentation is a major contributor to poor software quality. A four level Software System Documentation Process Maturity Model and assessment procedure developed by the authors is described in [7]. This paper briefly reports the results of 26 assessments of software projects at 7 different organizations.
