Apathy is a demotivation syndrome common in neurodegenerative diseases and is fundamentally multidimensional in nature. Different methodologies have been used to identify and quantify these dimensions, which has resulted in multifarious concepts, ranging in the number and characteristics of apathy subtypes. This has created an ambiguity over the fundamental substructure of apathy. Here we review the multidimensional concepts of apathy and demonstrate that overlapping elements exist, pointing… Expand

Apathy: Neurobiology, Assessment and Treatment
  • M. Fahed, D. Steffens
  • Medicine
  • Clinical psychopharmacology and neuroscience : the official scientific journal of the Korean College of Neuropsychopharmacology
  • 2021
TLDR
The neuropsychology and associated neurobiological underpinnings of apathy in the above conditions are addressed, specific methods to assess apathy clinically are identified, and the literature on managing apathy across these various disorders is reviewed. Expand
Multidimensional Apathy in Behavioral Variant Frontotemporal Dementia, Primary Progressive Aphasia, and Alzheimer Disease
TLDR
The emergent apathy profile for bvFTD seems to be emotional apathy (indifference or emotional/affective neutrality), with lower self-awareness in this subtype than executive apathy, which differentiates bv FTD from PPA. Expand
The brief Dimensional Apathy Scale: A short clinical assessment of apathy
TLDR
The b-DAS is an informant/carer-based robust yet short multidimensional apathy instrument with good convergent and divergent validity, with recommended clinical cutoffs, appropriate for use in the clinic and for research to quickly and comprehensively screen for apathy subtype impairments. Expand
Apathy Is Correlated with Widespread Diffusion Tensor Imaging (DTI) Impairment in Amyotrophic Lateral Sclerosis
TLDR
The significant correlations between apathy and DTI measures in several brain areas may suggest that subtle WM changes may be associated with mild behavioral symptoms in ALS even in the absence of overt cognitive and behavioral impairment. Expand
Alterations of regional homogeneity in Parkinson's disease with "pure" apathy: A resting-state fMRI study.
TLDR
This study used ReHo for the first time to characterize "pure" apathy related regional spontaneous brain function within the frontostriatal circuits in Parkinson's disease. Expand
Recommendations for the Nonpharmacological Treatment of Apathy in Brain Disorders.
  • V. Manera, S. Abrahams, +19 authors P. Robert
  • Medicine
  • The American journal of geriatric psychiatry : official journal of the American Association for Geriatric Psychiatry
  • 2019
TLDR
Recommendations for the use of NPT for apathy are provided, and the role of ICT in this domain is discussed, based on opinions gathered from experts in the field. Expand
Beyond language impairment: Profiles of apathy in primary progressive aphasia
TLDR
The Dimensional Apathy Scale is the first to reveal evidence of the dimensional nature of apathy in PPA, with different clinical signatures observed for each subtype, and the results will inform the development of targeted interventions for specific aspects of apathetic which emerge in P PA. Expand
Abnormal Spontaneous Neural Activity in Parkinson’s Disease With “pure” Apathy
  • Hai-Hua Sun, Jian-Bin Hu, +4 authors Chun-feng Liu
  • Medicine
  • Frontiers in Neuroscience
  • 2020
TLDR
The functional pattern of changes in spontaneous neural activity in patients with PD-A was characterized to better elucidate the pathophysiological mechanisms responsible for these changes, and apathy severity was negatively correlated with the ALFF in the bilateral nucleus accumbens and dorsal ACC in the pooled patients withPD. Expand
Reliability and validity of the brief dimensional apathy scale.
TLDR
The b-DAS is a fast, reliable, and valid instrument for screening apathy subtypes independent of physical disability and shows good internal consistency, excellent test-retest reliability, and significant positive correlation with the original DAS. Expand
The Association Between Impaired Awareness and Depression, Anxiety, and Apathy in Mild to Moderate Alzheimer's Disease: A Systematic Review
TLDR
High-quality studies suggested that in people with early AD, impaired awareness is related to fewer depressive and anxiety symptoms and to more apathy, which is a protective factor against experiencing affective symptoms in AD. Expand

Apathy in Alzheimer's Disease
TLDR
Assessment of apathy in Alzheimer's disease requires clinicians to distinguish loss of motivation from loss of ability due to cognitive decline, and current research has shown apathy to be a discrete syndrome. Expand
Multidimensional apathy in ALS: validation of the Dimensional Apathy Scale
TLDR
Apathy in ALS is characterised by a specific profile of increased initiation apathy and reduced emotional apathy, and the new Dimensional Apathy Scale (DAS) is a reliable and valid measure for the assessment of multidimensional apathy. Expand
The anatomy of apathy: A neurocognitive framework for amotivated behaviour
TLDR
It is argued that motivated behaviour requires a willingness to work, to keep working, and to learn what is worth working for, and that deficits in any one or more of these processes can lead to the clinical syndrome of apathy. Expand
Apathy: a neurocircuitry model based on frontotemporal dementia
TLDR
It is argued that bvFTD constitutes a disease model to study the neurocircuitry of complex behaviours as a ‘lesion-based’ approach to neuropsychiatric symptoms observed across diagnostic categories. Expand

TLDR
Clinical, neuropathologic, and neuroimaging observations increasingly suggest that apathy reflects dysfunction of frontal-subcortical circuits, especially those linking the ventromedial prefrontal cortex to related regions in the basal ganglia. Expand
Behavioural and emotional symptoms of apathy are associated with distinct patterns of brain atrophy in neurodegenerative disorders
TLDR
Findings provide further support for the role of medial frontal regions and insular cortex in apathy and suggest that behavioural and emotional aspects of the apathy syndrome may have distinct neuroanatomical bases. Expand
Multidimensional apathy and executive dysfunction in amyotrophic lateral sclerosis
TLDR
This is the first study to show specific associations between apathy subtypes (Emotional and Initiation) and executive and emotional cognitive dysfunction, indicating possible distinct underlying mechanisms to these demotivational symptoms. Expand
Developing a new apathy measurement scale: Dimensional Apathy Scale
TLDR
The DAS is a well-constructed method for assessing multidimensional apathy suitable for application to investigate this syndrome in different disease pathologies. Expand
Dimensions of apathy in Parkinson's disease: exploratory factor analysis of the apathy scale.
TLDR
The three factor solution resembled the triadic structure of cognitive, behavioral, and affective domains of apathy in Parkinson's disease; however, adjustments to the AS may be appropriate. Expand
Apathy in Parkinson's disease: clinical features, neural substrates, diagnosis, and treatment
TLDR
Parkinson's disease can be viewed as a model that provides insight into the neural substrates of apathy, and the opportunity to manipulate dopaminergic treatment in Parkinson's disease allows researchers to study a continuous range of motivational states. Expand
