Diagnostically, headache is the easy part of migraine. It is the surrounds of migraine—the aura, prodrome and postdrome—that can be most challenging, and confused with other pathologies. This article examines the definition and variants of migraine; alternative diagnoses for which migraine may be mistaken (mimics); conditions that lie between migraine and other diagnoses (borderlands) and the possible presentations of migraine posing as other conditions (chameleons). The focus is on adults…

TLDR
The author discusses the differential diagnosis of acute headache, especially thunderclap headache, and main strategies of effective treatment of migraine attacks.
Migraine Mimics Are we underdiagnosing migraines ?
TLDR
The diagnosis of migraine mimics is considered at the time of the initial consultation, starting with primary headache disorders and then moving on to secondary headache disorders, and later in the course of treatment when people’s headaches do not respond to treatment as expected or when headache patterns change.
Pediatric Episodic Migraine with Aura: A Unique Entity?
TLDR
The purpose of this review is to highlight the current evidence demonstrating the unique pathophysiology, clinical characteristics, differential diagnosis, co-morbidities, and treatment recommendations and responses for pediatric MA.
Preictal Symptomatology in Migraines: Diagnostic and Therapeutic Significance
TLDR
The clinical characteristics of diagnostic and therapeutic significance of preictal symptoms and various types of auras in migraine can be substantially increased by including prodromal physical signs in children with migraines.
[Clinical management of migraine].
TLDR
The migraine is historically one of the most studied primary headaches and there are many physiopathological mechanisms that explain the migraine aura.
First seizures in adults
TLDR
This article focuses on the diagnosis of first seizures and differentiation from their “mimics” in adults, and examines randomised controlled trials, systematic reviews, and meta-analyses.
Cryopyrin-Associated Periodic Fever Syndrome and the Nervous System
TLDR
CAPS is a highly debilitating disorder characterised by unregulated IL-1β production driven by autosomal dominantly inherited mutations in the NLRP3 gene, and effective therapies targeted against IL- 1 are now available and are vital to prevent long-term complications.
Bidirectional association between migraine and rheumatoid arthritis: two longitudinal follow-up studies with a national sample cohort
TLDR
Migraine increases the risk of RA, and RA is also associated with an increased risk of migraine.

TLDR
This review explores the mimics of adult epilepsy, the epilepsy borderland, and focuses particularly on epilepsy chameleons, which include syncope and psychogenic attacks.

TLDR
It is not uncommon for a patient with a lifetime’s history of migraine to experience a typical bout of cluster headache, although the reverse is less common.
Acute confusional migraine: our knowledge to date
TLDR
The confusional state often manifests with a wide diversity of cortical dysfunctions, such as speech difficulties, increased alertness, agitation and amnesia, and may well be underdiagnosed.
Sensory “pseudoperipheral” migraine aura
TLDR
This study reports two patients with typical migraine attacks that were preceded by sensory symptoms affecting one lower limb with a ‘‘pseudoperipheral’’ distribution, and reports the first symptoms resembled those of L5 and S1 radicular lesions.
Thunderclap headache
TLDR
The various causes of thunderclap headache are described and the role of investigations when CT and lumbar puncture investigations are normal are described, to help identify those who require urgent investigations.
Migraine With Aura and Migraine Without Aura: An Epidemiological Study
TLDR
The present findings suggest that MA and MO share the pain phase, and headache was, however, less severe and shorter lasting in MA than in MO.
Late-life migraine accompaniments: A narrative review
TLDR
Late-life onset of migraine with aura is not rare in clinical practice and can occur without headache, especially in elderly individuals, and treatment is limited and still inconsistent.
A nosographic analysis of the migraine aura in a general population.
TLDR
The intra-individual variation of aura symptoms shown in this study indicates that a simplification of the International Classification of Diseases, Neurological Adaptation is appropriate and that cortical spreading depression is the mechanism underlying the migraine aura.

TLDR
The development of allodynia suggests the withdrawal of inhibition of the afferent inflow from cutaneous receptors in line with that from cranial arteries causing the pulsatile nature of the headache, and from the special senses responsible for enhanced susceptibility to light, sound and smells.
