The debate on the metabolic syndrome has attracted great interest among physicians and researchers. This article sheds new light on the term by using a tool called causal diagrams (also known as causal directed acyclic graphs). Formal analysis according to causal and statistical principles reveals little substance behind the new syndrome, as well as numerous false claims. From a research viewpoint, continued use of a variable called 'metabolic syndrome status' should be discouraged.
Defining Metabolic Syndrome: Which Kind of Causality, if any, is Required?
TLDR
It is argued that the controversy over MetS is at least partially due to the different ‘causal content’ of the various definitions: their theoretical validity and practical utility can be evaluated by reconstructing or making explicit the underlying causal structure.
Causal diagrams and change variables.
TLDR
A true change is not a variable in the physical world, and modelling the change between two time points is justified only in a few situations.
Causal diagrams, information bias, and thought bias
TLDR
It is shown that measurement is a complex causal process that has two components, ie, imprinting and synthesizing, and a previously unrecognized bias is described, which may be viewed as antithetical to information bias.
Composite Risk Scores
TLDR
This review will describe the development and clinical utility of the Framingham Risk Score, the Reynolds risk score, the Pooled Cohort Equations, lifetime risk scores, the metabolic syndrome, the Edmonton Obesity Staging System, and the Cardiometabolic Disease Staging system.
The HEALS Project
TLDR
The HEALS project aims at identifying the complex links among genes, environment, and human disease on allergies and asthma, neurodevelopmental/neurodegenerative and metabolic disorders based on individual exposome characterization and how should this be implemented in large cohorts.
Unraveling the Exposome: A Practical View
TLDR
Comprehensive data on the internal, external, and public health components of the exposome together could inform risk assessment and ultimately guide risk management.

TLDR
The conclusion of two recent studies is that there is no added predictive value of the metabolic syndrome when the individual components are in the model.

Causal diagrams for epidemiologic research.
TLDR
Causal diagrams can provide a starting point for identifying variables that must be measured and controlled to obtain unconfounded effect estimates and provide a method for critical evaluation of traditional epidemiologic criteria for confounding.
Metabolic syndrome: nothing more than a constellation?
TLDR
It is proposed that a set of metabolic and cardiovascular risk factors—such as hypertension, hypertriglyceridaemia, low high-density lipoprotein (HDL)-cholesterol levels, and hyperinsulinaemia—could have a common aetiology based on insulin resistance, this latter also playing per se a pivotal role on pathophysiology of CVD.
The metabolic syndrome: more than the sum of its components?
TLDR
Given the high prevalence of the metabolic syndrome, studying its association with stroke is certainly relevant for 2 reasons: first, to obtain a better etiologic understanding of the causes of stroke; second, to identify individuals at high risk for stroke.
Can DAGs clarify effect modification?
TLDR
This work suggests an enhancement in construction of DAGs in epidemiology that includes arrow-on-arrow representations for effect modification, and illustrates possible biologic implications for such an effect modification DAG.
Metabolic Syndrome: Is It a Syndrome? Does It Matter?
To provide useful answers to the title questions, the first issue to resolve is what is meant by the term “metabolic syndrome.” If it is supposed to refer to a loose clustering of signs that are
Metabolic syndrome: what's in a name?
TLDR
This paper looks at the current definitions of metabolic syndrome and the bearing it has on clinicians both now and in the future.
