Despite refinements in chemo-radiotherapy regimens for first-line treatment of anal cancer (Glynne-Jones, chapter 4), 20–25% of cases will have local disease relapse, mainly during the first 3 years [1,2]. For these patients, salvage surgery offers the only opportunity for cure.
