}BACKGROUND
Up to one-third of patients with an inguinal hernia have no symptoms from the hernia. [] Key MethodMETHODS
Some 160 men aged 55 years or more with a painless inguinal hernia were randomized to observation or operation between 2001 and 2003. All were invited to attend a research clinic at 6 and 12 months, and 5 years after randomization. Those unable to attend for clinical review were sent a questionnaire based on the clinical review pro forma.

Watchful waiting is an option for men with minimally symptomatic or asymptomatic inguinal hernias but patients must be informed that there is a high risk of becoming symptomatic.

The long-term results of this trial indicate not enough evidence for differences in recurrence, chronic pain, and sensibility disorders betweenMesh plug repair and Lichtenstein's operation but a lower likelihood for reoperation for mesh plug repair.

Men who present to their physicians because of an inguinal hernia even when minimally symptomatic should be counseled that although WW is a reasonable and safe strategy, symptoms will likely progress and an operation will be needed eventually.
