In response to recently published KDIGO (Kidney Disease: Improving Global Outcomes) guidelines for the care of kidney transplant recipients (KTRs), the National Kidney Foundation's Kidney Disease Outcomes Quality Initiative (KDOQI) organized a working group of transplant nephrologists and surgeons to review these guidelines and comment on their relevance and applicability for US KTRs. The following commentaries on the KDIGO guidelines represent the consensus of our work group. The KDIGO…

TLDR
It is suggested that medical/surgical societies involved in the transplantation care spectrum convene a multi-disciplinary group of experts to create a new section in the kidney transplantation guidelines addressing the collaborative management of parathyroid disease in transplantation candidates.
Considerations for a Primary Care Physician Assistant in Treating Kidney Transplant Recipients.
TLDR
PAs must remain cognizant of the emotional well-being of the KTR, as many transplant patients struggle with fear, frustration, and acceptance, and the crucial components and considerations for PAs caring for kidney transplant recipients are provided.
Post-Transplant Diabetes Mellitus: Causes, Treatment, and Impact on Outcomes.
TLDR
Delays screening and diagnosis of PTDM are recommended until the recipient is on stable doses of immunosuppression after discharge from initial transplant hospitalization, and hemoglobin A1C is cautioned that it is not reliable as the sole diabetes screening method during the first year after transplant.
A Comprehensive Review of Infections in Older Kidney Transplant Recipients
TLDR
KT can be a successful treatment for older adults on dialysis if post-transplant complications, including rejection and infection, can be appropriately managed and the most recent literature and guidelines in the field of kidney transplantation are reviewed.
Hypertension after kidney transplantation
TLDR
It is hypothesised that changes in sympathetic nerve activity assessed by renal ¹²³I-mIBG scintigraphy, relate to changes in BP and catecholamines after catheter-based renal sympathetic denervation (RDN), and that incomplete denervation is a possible explanation for the lack of RDN efficacy.

TLDR
Although several guidelines make recommendations for cancer screening in SOTRs, the availability of cancer screening recommendations varied considerably by transplanted organ, and guideline development should involve transplant patients, oncologists and cancer screening specialists.

These guidelines, which are a modified version of the K/DOQI dyslipidemia guidelines, were developed to aid clinicians in the management of dyslipidemias in kidney transplant patients and are supported mainly by data from studies in the general population but there is an urgent need for additional studies in CKD and in transplant patients.

Results varied between studies but suggested that CNI withdrawal is safe and that conversion to MMF or Rapa may be beneficial, but did not result in a consensus regarding the management of CAN and CAD.
