}The application of a number of procedures that can be considered intraoperative endovascular neurosurgery has enhanced our ability to treat cerebral aneurysms from the abluminal surface. This study identifies a role for these techniques in the management of difficult aneurysms. A review of the last 1202 aneurysms undergoing direct clipping by the authors disclosed that these methods were used in 62 cases. Of these aneurysms, 36 arose from the internal carotid artery, 12 from the middle cerebral…Â

Endovascular balloon occlusion and suction decompression did not increase the complication rate in a large cohort of craniotomy patients with ophthalmic aneurysms, suggesting this technique may be used to augment surgical capabilities without significantly increasing the operative risk.

Endovascular management of complex intracranial aneurysms is increasingly being considered as an alternative to standard surgical clipping and management strategies that combine operative and endovascular techniques in a complementary way can be designed accordingly.
Giant aneurysms of the middle cerebral artery trifurcation treated with extracranial-intracranial arterial bypass and endovascular occlusion. Report of two cases.
- MedicineJournal of neurosurgery

Two patients with giant aneurysms of the MCA trifurcation benefited from flow inversion treatment and were clinically intact during the postoperative course, and follow-up angiography performed 11 and 4 months, respectively, after vessel occlusion showed that theAneurysm Occlusion was stable.

Endovascular treatment of wide-necked cerebral aneurysms can be facilitated by simultaneous temporary balloon protection by allowing more dense intra-aneurysmal coil packing, especially in the neck, without parent artery compromise.

It is suggested that this technique is a viable alternative in the management of patients with posterior fossa aneurysms associated with high surgical risk and longer angiographic and clinical follow-up study is necessary to determine the long-term efficacy of this recently developed endovascular occlusion technique.

Several parameters were found to be related to the postoperative development of ischemic injury, including the duration and nature of the temporary arterial occlusion, the number of the occlusive episodes, the specific vascular territory occluded, patient age, neurological status, presence of subarachnoid hemorrhage, vasospasm, and aneurysm size.

Recurrent ICA-PCoA aneurysms were separated into two types based on the position of the old clip in relation to the new growth, which is very useful from a surgical point of view because it is possible to apply a new clip without removing theold clip, which was found to be adherent to surrounding tissue.

Ophthalmodynamometry has provided a practical means of estimating the relative pressure decrements in the internal carotid following postocclusion and measured by direct cannulation the intravascular pressures in distal arteries at craniotomy to find that the pressure reductions beyond the bifurcation of the internal Carotid were equivalent to those obtained in the cervical portion of the carotids immediately distal to the site of occlusion.
