Purpose of Review: The purpose of this review is to address bacterial, viral, and other infectious causes of neuropathy or neuronopathy, with an emphasis on clinical manifestations and treatment. Recent Findings: Most infectious neuropathies have been well described for some time and treatments are well established. An exception is HIV-associated distal symmetric polyneuropathy, which is an area of active research. Current work in this area focuses on epidemiology, risk factors, and underlying…

The multifocal necrotic endoneurial nerve lesions with polymorphonuclear cell infiltration the authors describe may help identify cytomegalovirus neuropathy when characteristic inclusions are not present in the biopsy specimen.

An overlapping spectrum of meningitis, encephalitis, and myeloradiculitis occurs in CNS WNV infection, and fever, rash, abdominal and back pain, preceding a proximal, asymmetric flaccid weakness, with CSF pleocytosis help distinguish the motor syndrome from Guillain–Barré syndrome.

A progressive inflammatory neuropathy in swine slaughterhouse workers that appears to be a toxin-induced immune response is described and dichloroacetate is a promising agent for lactic acidosis-associated disorders, but toxic neuropathy is a treatment-limiting factor.
HIV neuropathy natural history cohort study

In this cohort of advanced HIV infection–infected subjects, distal sensory polyneuropathy was common and relatively stable over 48 weeks, and previously established risk factors, including CD4 cell count, plasma HIV RNA, and use of dideoxynucleoside antiretrovirals were not predictive of the progression of DSP.
Nerve damage in leprosy and its management
- Medicine, BiologyNature Clinical Practice Neurology
- 2008
Evidence that clinical neuropathy represents only the 'tip of the iceberg' of nerve damage, together with an understanding that the underlying inflammatory immunological processes require longer, more-aggressive treatment, could help usher in new and more-successful approaches to the treatment of nerve-function impairment in leprosy.
