Rapid-onset obesity with hypothalamic dysfunction, hypoventilation, and autonomic dysregulation (ROHHAD) is a rare and complex pediatric syndrome, essentially caused by dysfunction of 3 vital systems regulating endocrine, respiratory, and autonomic nervous system functioning. The clinical spectrum of ROHHAD is broad, but sleep/wake disorders have received relatively little attention so far, although the central hypothalamic dysfunction would make the occurrence of sleep symptoms likely. In this…
Narcolepsy during Childhood: An Update.
TLDR
Narcolepsy type 1 indeed is a lifelong disorder with a devastating impact on quality of life, especially when arising across developmental age, and targeted school programs, medicolegal and psychological supports are essential for patients care.
Anti-Hypothalamus and Anti-Pituitary Autoantibodies in ROHHAD Syndrome: Additional Evidence Supporting an Autoimmune Etiopathogenesis
TLDR
Findings support the involvement of autoimmunity in ROHHAD syndrome, but response to immunosuppressive treatment was only transient and the patient died of cardiorespiratory arrest.
ROHHAD Syndrome, a Rare Cause of Hypothalamic Obesity: Report of Two Cases
TLDR
Clinical and biochemical findings such as respiratory and circulatory dysfunction, hypothalamic hypernatremia, central hypothyrodism, hyperprolactinemia and central early puberty in patients matched the criteria for ROHHAD syndrome, which should be considered in the differential diagnosis of monogenic obesity.
Sleep disorders and the hypothalamus.
ROHHAD (Rapid-onset Obesity with Hypoventilation, Hypothalamic Dysfunction, Autonomic Dysregulation) Syndrome—What Every Pediatrician Should Know About the Etiopathogenesis, Diagnosis and Treatment: A Review
TLDR
This work provides a comprehensive description of the etiopathogenetic theories of the disease, clinical presentation, diagnostic workup and treatment possibilities of the ROHHAD syndrome.
Symptomatic Narcolepsy or Hypersomnia, with and Without Orexin (Hypocretin) Deficiency
TLDR
This chapter provides an overview of cases of symptomatic narcolepsy and EDS and extends the discussions to the roles of the orexin system in EDS disorders associated with various neurological conditions.
ROHHAD(NET) Syndrome: Systematic review of the clinical timeline and recommendations for diagnosis and prognosis.
TLDR
The clinical timeline of symptoms of ROHHAD(NET) is described and guidance for systematic follow-up and multidisciplinary management is proposed with the aim of improving prognosis and life expectancy.
Rapid-onset obesity with hypothalamic dysfunction, hypoventilation, and autonomic dysregulation: review and update
TLDR
The hope is that, with growing recognition, discussion, and investigation into the overlap of ROHHAD with disorders outside congenital central hypoventilation syndrome, further advancement will be made.

Monozygotic Twins Discordant for ROHHAD Phenotype
TLDR
These cases confound the hypothesis of a monogenic etiology for ROHHAD and indicate alternative etiologies including autoimmune or epigenetic phenomenon or a combination of genetic predisposition and acquired precipitant.
The Pathophysiologic Basis of Secondary Narcolepsy and Hypersomnia
TLDR
It is found that several symptomatic narcoleptic cases with MS show unique bilateral symmetric hypothalamic lesions associated with significant hypocretin ligand deficiency, suggesting a new clinical entity.
Delineation of Late Onset Hypoventilation Associated with Hypothalamic Dysfunction Syndrome
TLDR
The cause underlying LO-CHS/HD remains poorly understood although recurrence in siblings argues for a monogenic disorder and the outcome remains poor for this group of patients.
Endocrine manifestations of the rapid-onset obesity with hypoventilation, hypothalamic, autonomic dysregulation, and neural tumor syndrome in childhood.
TLDR
Various hypothalamic-pituitary endocrine dysfunctions are associated with ROHHADNET, carrying a risk of misdiagnosis until other elements of the syndrome make it more easily recognizable.

Rapid-Onset Obesity With Hypothalamic Dysfunction, Hypoventilation, and Autonomic Dysregulation: Analysis of Hypothalamic and Autonomic Candidate Genes
TLDR
Evidence is provided that variation of the HTR1A, OTP, and PACAP genes are not responsible for ROHHAD, and this results represent a further step in the investigation of the genetic determinants of RohHAD.
Decreased cerebrospinal fluid hypocretin-1 levels near the onset of narcolepsy in 2 prepubertal children.
TLDR
The levels of hypocretin in the cerebrospinal fluid were reduced when measured 3 weeks and 2 months after the appearance of hypersomnia, before the presence of sleep-onset rapid eye movement periods was confirmed.
Rapid-Onset Obesity With Hypothalamic Dysfunction, Hypoventilation, and Autonomic Dysregulation Presenting in Childhood
TLDR
Positive PHOX2B sequencing results demonstrate that this entity is distinct from congenital central hypoventilation syndrome, and provides a comprehensive description of the clinical spectrum of rapid-onset obesity with hypothalamic dysfunction, hypventilation, and autonomic dysregulation.
Primary Alveolar Hypoventilation Syndrome (Ondine's Curse): Association With Manifestations of Hypothalmic Disease
TLDR
A 3½-year-old white male was in good health until nine months prior to admission, when he developed a voracious appetite and lethargy, and gradually became less active and was disinterested in play.
