The key for the optimal management of adults with cyanotic congenital heart disease is the understanding of the underlying anatomy and pathophysiology. Central cyanosis results from persistent venous to arterial mixing due to a right to left shunt at the atrial, ventricular or arterial level resulting in chronic hypoxemia.
