Cumyl-PEGACLONE was the first synthetic cannabinoid (SC) with a γ-carbolinone core structure detected in forensic casework and, since then, it has dominated the German SC-market. Here the first four cases of death involving its fluorinated analog, 5F-Cumyl-PEGACLONE, a recently emerged γ-carbolinone derived SC, are reported. Complete postmortem examinations were performed. Postmortem samples were screened by immunoassay, gas chromatography mass spectrometry (GC–MS) or liquid chromatography…
Emergence of Cumyl-PEGACLONE-related fatalities in the Northern Territory of Australia
TLDR
An autopsy case series of five Cumyl-PEGACLONE-related fatalities over a recent eighteen-month period reaffirm an association with underlying cardiovascular disease, and suggest that concurrent use with alcohol may be relevant.
In vitro activity-profiling of Cumyl-PEGACLONE variants at the CB1 receptor: fluorination versus isomer exploration.
TLDR
It is demonstrated that 5-fluorination of Cumyl-PEGACLONE is not linked to an intrinsically higher CB1 activation potential, and that the ethylbenzyl and n-propylphenyl isomers yield a strongly reduced CB1 activitation.
Post-Mortem Toxicology: A Systematic Review of Death Cases Involving Synthetic Cannabinoid Receptor Agonists
TLDR
Even after a comprehensive analysis of clinical, circumstantial, toxicological, and autoptic data, the cause and manner of death remain unclear in some cases.
The Novel Psychoactive Substance Cumyl-CH-MEGACLONE: Human Phase-I Metabolism, Basic Pharmacological Characterization, and Comparison to Other Synthetic Cannabinoid Receptor Agonists with a γ-Carboline-1-one Core.
TLDR
Comparisons to its analogs suggest that Cumyl-CH-MEGACLONE is more likely to resemble the pharmacologic profile of the latter one than 5F-Cumyl-PEGAClONE, which is also a novel SC based on a γ-carboline-1-one core structure.
Blood concentrations of synthetic cannabinoids
TLDR
The observed concentrations of SCs vary widely and often have overlapping ranges for fatal and non-fatal cases, therefore, a referenced compilation of concentration ranges would be useful.
NNL-3: A Synthetic Intermediate or a New Class of Hydroxybenzotriazole Esters with Cannabinoid Receptor Activity?
TLDR
The synthesis of NNL-3 analogues proved simple and general, and some of these showed potent cannabimetic profiles in vitro, indicating that HOBt esters of this type may represent an emerging class of SCRA NPS.
Synthetic Cannabinoids 5F-QUPIC and MDMB-CHMICA in Plant Material – Identification and Quantification by Gas Chromatography – Mass Spectrometry (GC-MS), Nuclear Magnetic Resonance (NMR), and High-Performance Liquid Chromatography with Diode Array Detection (HPLC-DAD)
Abstract The synthetic cannabinoids 5F-QUPIC and MDMB-CHMICA were sprayed on plant material intended for smoking and was seized as a criminal evidence. Their presence was confirmed by gas
Comprehensive analytical characteristics of N-(adamantan-1-yl)-1- (cyclohexylmethyl)-1H-indazole-3-carboxamide (ACHMINACA)
Purpose The aim of this study was to clarify the most essential analytical features of N -(adamantan-1-yl)-1-(cyclohexylmethyl)-1 H -indazole-3-carboxamide (one of the newest cannabimimetics called
Revisited: Therapeutic and toxic blood concentrations of more than 1100 drugs and other xenobiotics
TLDR
The data provided are useful for the therapeutic monitoring of pharmacotherapies, to facilitate the diagnostic assessment and monitoring of acute and chronic intoxications as well as to support forensic and clinical expert opinions.

Phase I metabolism of the recently emerged synthetic cannabinoid CUMYL-PEGACLONE and detection in human urine samples.
TLDR
The latest cumyl-derivative, CUMYL-PEGACLONE, emerged in December 2016 on the German drug market and features a novel γ-carboline core structure, which is most likely synthesized to bypass generic legislative approaches to control SCs by prohibiting distinct core structures.
Human phase I metabolism of the novel synthetic cannabinoid 5F-CUMYL-PEGACLONE
TLDR
5F-CUMYL-PEGACLONE was subject to extensive metabolism in humans and the propionic acid metabolite was the most abundant metabolite in all urine samples and should be targeted when maximum sensitivity is needed.
Testing for SGT-151 (CUMYL-PEGACLONE) and its Metabolites in Blood and Urine after Surreptitious Administration.
TLDR
A dealer gave surreptitiously a substance to two juvenile migrants who experienced shakings and faintness, who, according to the investigators, were probably exposed to SGT-151, a SC, also known as CUMYL-PEGACLONE.
Four postmortem case reports with quantitative detection of the synthetic cannabinoid, 5F-PB-22.
TLDR
Over the course of a 4-month time period (July-October 2013), the laboratory quantitatively identified 5F-PB-22 in specimens obtained from four postmortem cases, to include pertinent autopsy findings and decedent histories, together with quantitative results for5F- PB-22 determined in postmortem blood and antemortem serum.
Acute side effects after consumption of the new synthetic cannabinoids AB-CHMINACA and MDMB-CHMICA
TLDR
The valine derivative AB-CHMINACA and the tert-leucine derivative MDMB-CHMICA (“third generation of SCs”) seem to be associated with more severe clinical toxicity than was previously reported in patients exposed to earlier generation SCs such as JWH-018.
Determining the Toxicological Significance of Pregabalin in Fatalities
TLDR
Pregabalin, especially with concomitant use of other CNS depressant drugs, presents a significant toxicological risk and existing laboratory protocols should be reviewed for their suitability to detect pregabalin.
A fatal case involving several synthetic cannabinoids
TLDR
An acute influence of several synthetic cannabinoids and amphetamine can be assumed and detection of JWH-210 in adipose tissue and hair and lack of it in blood and joint indicates that a previous consumption of other substances had occurred.
A fatal case of MAM-2201 poisoning
TLDR
A 59-year-old man was found dead in his house, where three sachets containing herbal blends were found on a table and it was concluded that the man’s death was caused by acute intoxication with MAM-2201 and the distribution of the drug in postmortem human tissues and blood was described.
Postmortem distribution of AB-CHMINACA, 5-fluoro-AMB, and diphenidine in body fluids and solid tissues in a fatal poisoning case: usefulness of adipose tissue for detection of the drugs in unchanged forms
TLDR
The results suggest that adipose tissue is the best specimen for detection of lipophilic drugs, such as AB-CHMINACA, 5-fluoro-AMB, and diphenidine, in their unchanged forms in an autopsy case in which multiple drugs were judged as poisoning by multiple drugs.
