Several groups of investigators have described the presence of small fiber neuropathy in fibromyalgia patients. This writing discusses how this new finding could renovate fibromyalgia concept, diagnosis, and treatment. Predominant rheumatology thinking proposes fibromyalgia as a “centralized pain syndrome.” An alternative hypothesis views fibromyalgia as a stress-related dysautonomia with neuropathic pain features. Dorsal root ganglia may be the key autonomic-nociceptive short-circuit sites…
Is fibromyalgia an autoimmune illness?
TLDR
Fibromyalgia is clearly a stress-related disorder and only a minority of fibromyalgia patients displays clinically recognizable autoimmunity and/or auto-antibodies, so Goebel et al.

TLDR
A patient-centered holistic therapy aimed to regain autonomic nervous system resilience remains the most effective fibromyalgia therapy.
Complex Regional Pain Syndrome Evolving to Full-Blown Fibromyalgia: A Proposal of Common Mechanisms.
TLDR
Clinicians should be aware that CRPS can evolve to full-blown fibromyalgia and to discuss the potential pathogenetic mechanisms linking these debilitating illnesses.
Electrodiagnostic Abnormalities Associated with Fibromyalgia
TLDR
Electrodiagnostic data of both lumbar and sacral nerve root myotomes in fibromyalgia patients are presented for the first time and it is proposed that, in addition to skin punch biopsies to assess small fiber neuropathy, assessment of the anal reflex may be a useful part of the diagnostic pathway in patients with Fibromyalgia.
The puzzle of fibromyalgia between central sensitization syndrome and small fiber neuropathy: a narrative review on neurophysiological and morphological evidence
TLDR
Evidence based on structural and functional neuroimaging methods, electrophysiological, and morphological – skin biopsy – features demonstrated a central and peripheral nervous system involvement in fibromyalgia.
Fibromyalgia in women: somatisation or stress-evoked, sex-dimorphic neuropathic pain?
TLDR
Understanding fibromyalgia as a real stress-evoked neuropathic pain syndrome may lead to more compassionate patient care and may open new avenues for gender-related neuropathicPain investigation.
Correlation Between Corneal Nerve Density and Symptoms of Small Fiber Neuropathy in Patients With Fibromyalgia: The Confounding Role of Severe Anxiety or Depression
TLDR
Small fiber neuropathy and dysautonomia symptoms correlate with corneal denervation in women with fibromyalgia without severe anxiety or depression, and this clinical-pathological association reinforces fibromy nostalgia as a dysautonomyia-related neuropathic pain syndrome.
Unclear etiology and current hypotheses of the pathogenesis of fibromyalgia
TLDR
The newest finding of small fiber neuropathy in patients with fibromyalgia supports another hypothesis, in which the disease is presented as stress-related dysautonomia with neuropathic pain features.
High Prevalence of Perineural Cysts in Patients with Fibromyalgia and Chronic Fatigue Syndrome.
TLDR
The hypothesis that STCs, FM, and CFS may share the same pathophysiological mechanism, i.e., moderately increased cerebrospinal fluid pressure, causing irritation of neurons and axons in dorsal root ganglia, is supported.
Dorsal root ganglia: fibromyalgia pain factory?
TLDR
Novel neuroimaging technology and postmortem inquest may better define dorsal root ganglia involvement in fibromyalgia and similar maladies, as DRG pro-nociceptive molecules are attractive Fibromyalgia therapeutic targets.

Small fibre pathology in patients with fibromyalgia syndrome.
TLDR
A reduction in dermal unmyelinated nerve fibre bundles was found in skin samples of patients with fibromyalgia syndrome compared with patients with depression and with healthy control subjects, whereas myelination nerve fibres were spared.
Small nerve fiber involvement in patients referred for fibromyalgia
TLDR
A subset of FM subjects have SFN, which may contribute to their sensory and autonomic symptoms, and skin biopsy should be considered in the diagnostic work‐up of FM.
What is the meaning of "small fiber neuropathy" in fibromyalgia?
TLDR
There are likely differing underlying pathogenic mechanisms responsible for the small fiber loss in FM compared with those with SFN because the patients with FM had reduced axonal diameter and some abnormalities in Schwann cells seen on electron microscopy that had been previously reported in FM but not in SFN.
Immunotherapy Prospects for Painful Small-fiber Sensory Neuropathies and Ganglionopathies
TLDR
The evidence of immune causality and the limited data regarding immunotherapy for small-fiber-predominant ganglionitis, regional neuropathy (complex regional pain syndrome), and distal SFPN are reviewed.
Evidence of Abnormal Epidermal Nerve Fiber Density in Fibromyalgia: Clinical and Immunologic Implications
TLDR
This work sought to determine the prevalence of small fiber neuropathy, as measured by epidermal nerve fiber density (ENFD), in a series of patients with FM and clinically healthy control subjects.
Fibromyalgia: When Distress Becomes (Un)sympathetic Pain
TLDR
In fibromyalgia, distress could be converted into pain through forced hyperactivity of the sympathetic component of the stress response system.
