OBJECTIVE To evaluate the 4-year risk of high intraocular pressure (IOP) and associated factors in a black population without glaucoma at baseline. DESIGN Population-based incidence study. PARTICIPANTS Random sample of Barbados residents aged 40 to 84 years. After 4 years, 3427 (85%) were reexamined. MAIN OUTCOME MEASURES Development of elevated IOP (using percentile-based definitions) among individuals with an IOP of 21 mm Hg or lower and no glaucoma at baseline.
