This trial will serve as a basis for further targeted screening and chemoprevention trials based on genotype as further genes are identified, and the lessons learned in IMPACT will be screening uptake in a high-risk male population, the psychological issues of screening men at higher risk of PCa, the utility of PSA in a higher risk population and the identification of new and better biomarkers.

An earlier diagnosis is essential, according to these data, especially for men at a "high risk" of this cancer, which will constitute an essential tool for more effective anti-cancer strategy.

Analysis of genes encoding key proteins involved in androgen biosynthesis and action led to the observation of a significant association between a susceptibility to prostate cancer and common genetic variants, such as those found in 5alpha-reductase type 2 and AR genes.

Germ-line mutations in BRCA2 may account for about 5% of prostate cancer in familial clusters, and both were seen in individuals whose age at diagnosis was very young and who were members of an affected sibling pair.

Although HPC1 accounts for only a small proportion of all families affected by hereditary prostate cancer, it appears to play a more prominent role in the subset of families with several members affected at an early age and with male-to-male disease transmission.

A polygenic model of prostate carcinogenesis is developing, focused around a series of genes involved in androgen biosynthesis, transport and metabolism, which can potentially provide a basis for targeting populations for screening interventions and for implementing primary preventive strategies.
