Transmission electron microscopy shows that in keratitis sicca the essential lesion of the corneal epithelium consists of a rupture of the cell membranes with formation of keratin filaments, which project exteriorly. Scanning electron microscopy shows that the cell surfaces become smooth and that the cells appear dark and without craters. The fundamental changes in keratitis sicca is an abnormal keratinization.
