Schaffner introduces four articles on clinical trials (by F. Gifford, J.B. Kadane, L. Kopelman, and D. Marquis) by providing an historical and methodological context within which to interpret them. He discusses the conflict between the individual and collective ethic facing the physician-investigator; ethical dilemmas in starting and stopping trials; and alternatives to the randomized trial, such as prerandomization and Kadane's Bayesian methodology being used at Johns Hopkins University.
