BACKGROUND:The use of the fiberoptic endoscope is a recent innovation in pituitary surgery. OBJECTIVE:To investigate the evidence of an operative learning curve after the introduction of endoscopic transsphenoidal surgery in our unit. METHODS:The first 125 patients who underwent endoscopic transnasal transsphenoidal surgery for pituitary fossa lesions between 2005 and 2007 performed by 1 surgeon were studied. Changes in a number of parameters were assessed between 2 equal 15-month time periods…

A Learning Curve of Endoscopic Transsphenoidal Surgery for Pituitary Adenoma

As the effectiveness of endoscopic surgery increases, the duration of surgery and postoperative hospital stay decrease, and in this study, the learning curve in endoscopic pituitary surgery was identified.

Minimally invasive endoscopic unilateral transsphenoidal surgery for pituitary adenomas.

Purely endoscopic unilateral endonasal transsphenoidal adenoma resection is minimally invasive surgery leading to a good rate of gross total tumour resection and it is associated with a low complication rate.

Endoscopic versus microscopic transsphenoidal pituitary adenoma surgery: a meta-analysis

The present study indicates that the endoscopic transsphenoidal approach is safer and more effective than microscopic surgery in the treatment of pituitary adenomas.

Endoscopic transsphenoidal surgery for non-functioning pituitary adenoma: Learning curve and surgical results in a prospective series during initial experience

NFPA surgery can be safe and efficient during the learning period, and Dedicated intensive learning, careful patient selection and multidisciplinary work are key to shorten the learning curve and achieve satisfactory results.

Transient Exacerbation of Nasal Symptoms following Endoscopic Transsphenoidal Surgery for Pituitary Tumors: A Prospective Study

Nasal morbidity following endoscopic transsphenoidal pituitary surgery is common, but transient, more so in the functioning subgroup, and improvements in nasal symptoms improve below baseline by 6 to 12 months, without the need for specific long-term postoperative interventions.

Effect of pituitary adenoma consistency on surgical outcomes in patients undergoing endonasal endoscopic transsphenoidal surgery

Fibrous PAs showed higher rates of postoperative hypopituitarism, permanent diabetes insipidus (DI) and postoperative treatments (hormonal treatment and radiotherapy) and the prognosis of the patient undergoing EET was higher than normal.

The slope of the learning curve in 600 consecutive endoscopic transsphenoidal pituitary surgeries

Although the slope of the learning curve is steeper earlier in a surgeon’s experience, the slope does not plateau and continues to increase even over more than a decade.

The Learning Curve in Minimally Invasive Pituitary Surgery

This finding supports the concept that an otolaryngology/neurosurgery team skilled in endoscopic techniques and pituitary surgery can safely transition from open approaches to an endoscopic approach in pituitsary surgery.

The Endoscopic versus the Traditional Approach in Pituitary Surgery

The transsphenoidal route is the most widely used technique for pituitary adenoma surgery due to its rapidity, good tolerance, effectiveness and low complication rate, but tumor removal was notably superior when dealing with tumors having an extrasellar extension.

The endonasal transsphenoidal surgery under the neuroendoscope-assisted appears to be a safe, effective and micro-invasive method for PA.

Endoscopic endonasal transsphenoidal approach: outcome analysis of 100 consecutive procedures.

The endoscopic endonasal transsphenoidal approach appeared to be less traumatic than the traditional microsurgical approach, was very effective, and was characterized by a reduced number of complications, however, the relatively small series together with the short follow-up do not allow us to draw definitive conclusions.

Endoscopic pituitary surgery

Concerning results in terms of mass removal, relief of clinical symptoms, cure of the underlying disease and complication rate, they are, at least, similar to those reported in the major microsurgical series, but patient compliance is by far better.

Comparison of endoscopic and microscopic removal of pituitary adenomas: single-surgeon experience and the learning curve.

Outcomes after either purely endoscopic resection or traditional microscope-aided resection for resection of pituitary lesions of the sella turcica are compared to determine the learning curve associated with a surgical team converting to endoscopic techniques.

Endoscopic endonasal transsphenoidal surgery: experience with 50 patients.

Postoperatively, all patients who had undergone endonasal endoscopic surgery had unobstructed nasal airways with minimal discomfort and more than half of the patients required only an overnight hospitalization.

Surgical complications associated with the endoscopic endonasal transsphenoidal approach for pituitary adenomas.

Overall, a decreased incidence of complications has been observed, compared with large historical series of the traditional microsurgical transsphenoidal approach, likely because of the overview inside the anatomy facilitated by the endoscope, and the decreased surgical trauma.
