We present a 77-year-old female with recent myocardial infarction complicated by antero-apical ventricular septal defect (VSD) and quick development of low cardiac output. Using interrupted horizontal mattress sutures VSD closure was successfully performed on the beating heart. Transthoracic echocardiography at the four-month follow-up did not reveal residual VSD.
Outcome Analysis After Surgical Management of Ventricular Septal Defect Complicating Acute Myocardial Infarction
TLDR
Delayed elective repair of PIVSD is advocated in patients with CS who responded to aggressive conservative management maintaining hemodynamic stability, to allow inflammatory state to subside and in those patients with severe CS, additional rescue therapy with temporary mechanical circulatory support is needed to prevent further deterioration of systemic perfusion.
Revising ventricular septal defect residual shunts without aortic re-cross-clamping: a safe and effective surgical procedure.
TLDR
On beating heart surgery is a safe and effective approach for the closure of residual VSDs and is significantly associated with a lower incidence of prolonged ventilation, a lower rate of prolonged intensive care unit (ICU) stay, and reduced in-hospital expenses.
SURGICAL TREATMENT OF POST INFARCTION VENTRICULAR SEPTAL RUPTURE AND LEFT VENTRICULAR ANEURYSM WITHOUT AORTIC CROSS CLAMPING
TLDR
Discussion on the ICD of Cardiac Rupture
TLDR
Coders should fully understand the relevant medical knowledge of cardiac rupture, pore over records before coding, accurately distinguish the location, predisposing factors and clinical manifestations of cardiac ruptures, master ICD coding principles of cardiac Breach (CR), and carefully follow coding steps to achieve accurate coding.

TLDR
The clinical findings, catheterization data, and operative results suggest that closure of the rupture should be delayed when possible from three to six weeks after the infarction to allow firm fibrous healing of the region.

TLDR
The surgical mortality for treating patients with postinfarction VSD has decreased with improvements in surgical technique, and rapid diagnosis, appropriate preoperative management and delicate surgical repair improve the overall results and help to attain long-term survival.

An unusual case of RAG2 gene mutation induced PID presenting clinically as severe combined immunodeficiency (SCID) phenotype rather than Ommen’s syndrome, and acquired CMV infection via breast milk is described.
