}ABSTRACT There has been recent concern about the association between high dose methadone and prolongation of QTc in the electrocardiogram. QTc is the time from the beginning of the QRS complex to the end of the T have as measured on an electrocardiogram and corrected for heart rate. To date, no association has been made between methadone and buprenorphine in commonly used doses and prolonged QTc. Electrocardiograms were performed on groups of methadone (n = 35, mean daily dose ± standard…

Variability of QT Interval Measurements in Opioid‐Dependent Patients on Methadone
- Medicine
- 2014
Accurate identification of QT prolongation in methadone patients is difficult because of intrinsic patient variability in QT length and accuracy of automated measurements, which are best overcome with manual and serial QT measurements, especially at doses exceeding 100 mg/day, and interpretation using a QT nomogram to determine risk of ventricular dysrhythmia.
Methadone maintenance, QTc and torsade de pointes: who needs an electrocardiogram and what is the prevalence of QTc prolongation?

Over half to three-quarters of methadone maintenance patients fulfilled MHRA criteria for ECG monitoring, which has costly implications, and Methadone dose and stimulant use were associated with longer QTc intervals.

Infection with hepatitis C increases the likelihood of critical QTc prolongation in patients in methadone maintenance treatment, including development of torsade-de-pointes arrhythmia and death.

Opioids and Cardiac Arrhythmia: A Literature Review
- MedicineMedical Principles and Practice
- 2018
Methadone has a higher capability to induce long QT interval and dangerous arrhythmias in conventional doses than others and high doses of opioids must be used cautiously with periodic monitoring of ECG in high-risk consumers such as patients under opioid maintenance treatment.
cardiotoxicity of Oral Methadone as an Analgesic— recommendations for safe Use

An overview of the cardiotoxicity of oral methadone is provided, with an emphasis on its use as an analgesic, and a series of recommendations are provided to improve the cardiac safety profile of oral Methadone used for analgesia.

The present review aims at perusing the available literature about QT-related cardiac safety during methadone treatment for heroin addiction to find out whether conditions at higher risk of multiple and polydrug treatments deserve some greater surveillance.
